水网地区日本血吸虫感染及肝脾超声检查

【摘要】 目的 了解水网地区日本血吸虫病流行程度及流行特征。方法 用整群抽样法从湖北省江陵县某行政村抽取 5个村民小组共 76 9人为研究对象 ,用改良加藤法和尼龙绢集卵孵化法检测感染 ,用 B超探查肝脾。结果 总感染率为 44 .3% ,男女性感染率、感染度基本相同 ;从事农业生产为最主要感染方式 ,学生的感染率低于农民 ,但感染度与农民相同 ;感染者的肝脾肿大发生率高于非感染者 ,肝左叶厚与脾厚两项指标增大的发生率随感染度增加而上升。肝实质级数与感染血吸虫的次数呈直线相关关系。结论 该地区日本血吸虫病严重流行 ,B超检测结果与血吸虫现感染及过去史有联系

【Abstract】 Objective To study epidemiologic features and degree of endemicity of Schistosomiasis Japonica in water net region.Methods 769 individuals in 5 nature villages,Jiangli County,Hubei Province were selected in a cluster sampling study.Kato Katz method and hatching test were used to detect infection.The ultrasonographic eqipment was used in this study for observing liver and spleen images.Results Overall prevalence rate was 44.3% .There were no significant difference in the prevalence and intensity of Schistosoma Japonicum infection between males and females (P>0.05).Farmers had the highest prevalence (50.0%)among all occupational groups,though students with lower incedence showed similar worm intensity as farmers.Subjects infected with Schistosoma Japonicum had significantly higher percentages of liver and spleen enlargement.as compared with those of the uninfected (P<0.05).The incidences of hepatomegaly with increasing thickness of its left lobe and spleomegaly with increasing thickness increased with its intensity of infections.The grades of hepatomegaly shown by ultrasonographic liver images in Schistosoma Japonicum infections had a linear regression relationship with the number of repeated infections of Schistosoma Japonicum.Conclusion This area belongs to a heavy endemic area of Schistosomiasis Japonica.Results of ultrasonic examinations of liver and spleen were related to the current and past history of Schistosoma Japonicum infections.The control of Schistosomiasis Japonica infections in water net regions in China remained a serious problem.
