【摘要】 目的 :探讨定制人工膝关节假体在膝关节周围恶性骨肿瘤保肢治疗中的临床效果。方法 :对19例股骨远端或胫骨近端恶性骨肿瘤行瘤段骨广泛切除、定制人工膝关节假体置换术。结果 :随访 1年 5个月～ 5年 ,膝关节活动度为伸 0° ,屈 90°～ 10 5°。 19例患者中发生假体周围积液和行走后踝关节痛各 1例 ,无感染、假体松动及断裂 ,无肿瘤复发。 1例发生肺转移 ,带瘤存活 ,2例因肺及其他部位转移死亡。结论 :定制人工膝关节假体治疗膝关节周围恶性骨肿瘤能尽早及较好地恢复膝关节功能 ,尤其适合于骨破坏严重 ,骨支架作用基本丧失者。更多还原

【Abstract】 Objective:To investigate the clinical results and value of custom made artificial total knee prosthesis in limb sparing surgery for malignant bone tumors around the knee joint.Methods:19 cases of malignant bone tumors of the distal femur or the proximal tibia were treated by wide excision and total knee replacement.Results:Of all cases,the follow up period varied from 17 to 60 months.The extension and flexion spheres of knee activity were respectively 0° and 90°～105°.There were 1 case for effusion around prosthesis,1 case for ankle pain when walking and 3 cases for lung metastasis (2 cases dead,1 case alive).No infection,prosthesis loosening or fracture,and local recurrence happened.Conclusion:Total knee replacement is an effective method for treating malignant bone tumors around the knee and recovering knee function,especially to patients with serious osteolytic destruction and deprivation of supporting frame of bone.更多还原
