【摘要】 目的 :探讨体外循环术中自体血经离心洗涤后回输对机体红细胞免疫功能、形态的影响。方法 :随机选取 30例心脏瓣膜替换患者 ,分为自体血洗涤回输实验组及对照组。动态检测两组患者术前、术后1、2 4、72h及 7d红细胞免疫功能、血浆游离血红蛋白含量、血常规 ,并观测洗涤处理后浓缩血及机血、库血内红细胞形态变化。结果 :实验组患者术后RBC免疫功能优于对照组 ,且恢复较早 ;体内血浆游离血红蛋白含量明显减低 ;RBC数量及红细胞体积比回升较快。镜检示洗涤处理后的RBC形态正常 ,结构完整、清晰。结论 :自体血洗涤回输可促进体外循环术中受损红细胞免疫功能尽早恢复 ,并能保护其形态结构 ,增加新鲜完整红细胞的数量。更多还原

【Abstract】 Objective:To investigate the effect of RBC washing on cellular quantity,morphology and function.Methods:30 consecutive patients undergoing valve replacement were randomly divided into 2 groups.In study group,after being centrifuged and washed,the losing blood was also transfused.In the control group,without any disposal,only blood in the extracoporal machine or bank blood,if necessary,was routinely transfused.Blood samples were drawn respectively before operation,1,24 72 hours and 7 days after operation.We examined the quantity and immunity of RBC,free Hb quantity and blood routine.Morphologic changes of disposed blood,bank blood and residal blood were also observed.Results:In the study group,RBC C3b receptor rate,as well as immune compound rate,was lower than that in the control group at 1 and 24 hours time points(P<0.05),the quantity of plasma free Hb was also lower(P<0.05).Morphologic study showed the disposed RBC was distinct and intact.Conclusion:RBC washing and autotransfusion may improve the impaired RBC recovery,protect RBC morphology and structure.更多还原
