急性脑梗塞超早期脑循环动力学改变与CT对比研究

【摘要】 目的 :应用脑循环动力学检测仪 ( CVA)探测脑梗塞超早期血液动力学改变 ,与 CT比较 ,探讨 CVA对脑梗塞超早期诊断价值。方法 :对 1 2 8例超早期和急性期脑梗塞患者同时进行 CVA和 CT检查 ,比较两者间及与对照组检测指标的差异和相关性。结果 :1 CVA与 CT检测结果有一致性 ( P<0 .0 5) ;CVA阳性检出率高 ( P<0 .0 0 0 1 ) ,超早期 CVA阳性率 95.1 6% ,CT为 1 6.1 3% ;2 CVA检查异常侧与 CT及临床查体异常侧具有较高一致性 ( P<0 .0 0 0 1 ) ;3非腔隙性梗塞、多发性梗塞及超早期梗塞患者 CVA异常显著。结论 :CVA检测对脑梗塞超早期诊断具有敏感性、定侧性 ,对疾病超早期治疗、预后评估等具有重要性。

【Abstract】 Objective:To investigate the value of CVA in diagnosis of hyperacute ischemic stroke compared with CT.Methods:CVA and CT were performed in 128 patients with hyperacute and acute ischemic stroke.Relative hemodynamic parameters were evaluated on the occluded side and compared with CT findings as well as the data from control subjects.Results:①The results of CVA detecting corresponded with CT scans (P<0.05);The positive rate of CVA(95.16%) was much higher than that of CT(16.13%) in patients with hyperacute ischemic stroke (P<0.0001).②CVA had good correspondence with CT and clinical examination in detecting the abnormal side(P<0.0001).③The parameters of CVA detecting in hyperacute infarction,multiple infarction and largevessel territorial brain infarction had more serious abnormity than that in acute single infarction and lacunar lesions.Conclusions: These results show that CVA is more sensitive than CT in detecting hyperacute ischemic stroke and CVA parameters are identical with early CT findings.These findings may improve the diagnosis of ischemic stroke and may contribute to advances in treatment and prognosis.
