【摘要】 目的 :探讨功能性扩大根治术的临床效果及对术后早期膀胱和性功能的影响。方法 :对 189例直肠癌患者行广泛清除直肠上方 ,侧方及下方淋巴结并保留术野分布的植物神经。并与同期 2 94例直肠癌Miles术者比较生存率及膀胱和性机能。结果 :本组无死亡率 ,5、10年生存率分别为 61 9%、4 2 86%。术后膀胱机能障碍、勃起障碍、射精障碍分别为 18 2 %、15 34 %、18 52 %。结论 :该术式明显提高生存率 ,减少膀胱及性机能障碍的发生率 ,应是直肠癌根治术中的主要术式。更多还原

【Abstract】 Objective:To explore the clinical effects on bladder on sexual functiona by extending radical operation on rectal carcinoma.Methods:189 Cases of rectal caicinoma were treated with extensive clearance of the superior,laleral and the inferior of the lymph nodes of the rectum to protect the vegetative nerves which spread on the operation area. The survival rate and the bladder and sexual function of the above operations were compared with those of the Miles operations on 294 patients with rectal carcinoma.Results:No death was found in this group.The five year and ten year survival rates were 61 9% and 41.80%. The percentage of the patients with post operative vesical dysfunction, erection disorder and ejaculation disorder were 18.2%,15.34% and 18.52% respectively.Conclusions:There is obvious increase of survival rate and decrease of the incidence of vesical dysfunction and sexual disorder due to this operation and it should be regarded as the main type of operation on rectal carcinoma.更多还原
