【摘要】 目的 探讨十二指肠腺癌的CT表现,并与低张十二指肠造影(HD)比较,评估各自的临床价值。方法 回顾性分析16例经病理证实的十二指肠癌的CT和X线表现。结果 主要CT征象为腔内肿块、肠壁增厚、坏死,4例示溃疡,例浸润至周围脏器,腹膜后淋巴结和肝转移各2例,2例未能鉴别肿块来源,由HD确定。X线所测肿瘤大小与手术结果基本符合,14例均见不规则充缺、粘膜破坏,7例示溃疡。结论 CT可显示腔内病变、腔外浸润及转移。HD仅能观察腔内异常,并有助于鉴别肿块来源。两者结合能对十二指肠癌作出一较全面的形像学分析。更多还原

【Abstract】 Objective To discuss CT findings of duodenal adenocarcinoma compared with HD for evaluating their clinical value. Methods CT and X -ray signs of 16 cases duodenal adenocarcinoma proved histologically were evaluated retrospectively. Results CT main signs were intraluminal mass,thickening of bowel wall,necrosis. Ulcerations were seen in 4 cases. Adjacent strutures wese invaded in 11 cases. Liver metastaes and retroperitoneal adenopathy were seen in 2 cases,re-spectivlyjn 2cases,a primary origin of the mass was not identifiable in the duodenum, which was defined by HD. The size of intraluminal tumor was measured by X - ray in basic correspondence with operations result. Irregular filling defect and mucosa destruction were seen in 14 cases and ulcerations in 7cases. Conclusions CT Scan can demonstrate intraluminal lesions,ex-traluminal involvement and metastase. HD allows only to view intraluminal abnormality and is helpful in differentiating the o-rigin of mass. Duodenal adenocarcinoma can be evaluated imaging more completely by CT combining with HD.更多还原
