【摘要】 目的　探讨急性心肌梗死 (AMI)病人补体系统是否被激活以及补体激活的程度与心肌缺血损伤范围的关系。方法　 1999年 4月至 2 0 0 0年 4月检测 37例AMI病人和 2 0名健康志愿者血浆中补体激活产物C4d、Bb和C5b 9水平和肌酸磷酸激酶 (CK)水平 ;AMI病人在入院时使用Killip分级法评价心脏泵功能 ,并在入院后 2周检测左室射血分数 (EF)。结果　AMI病人血浆C4d、Bb和C5b 9水平明显高于健康志愿者 (分别为 5 .11± 1.31和 2 .30± 0 .6 4,0 .6 4± 0 .2 1和 0 .18± 0 .0 4,0 .83± 0 .32和 0 .0 6± 0 .0 2 ,单位 :mg/L) ,AMI合并心脏泵功能衰竭病人C5b 9水平明显高于不合并心脏泵功能衰竭病人 (分别为 1.14± 0 .41和 0 .6 8± 0 .2 7,单位 :mg/L) ,血浆C5b 9水平与CK峰值呈正相关 ,与EF呈负相关。结论　AMI病人补体系统被激活 ,补体激活的程度与心肌缺血损伤范围相关。更多还原

【Abstract】 Objective To define whether the complement system is activated and whether there is the relation between degree of complement activation and myocardial damage in patients with actue myocardial infarction (AMI).Methods The plasma level of products of complement activation C4d、Bb and C5b 9 complexes,peak creatine phosphokinase (CK) and left ventricular ejection fraction (EF) were measured in 37 patients with AMI,and C4d,Bb and C5b 9 were also measured in 20 normal volunteers as the control.Results The plasma levels of C4d、Bb and C5b 9 in patients with AMI were significantly higher than those in normal volunteers (5.11±1.31 vs 2.30±0.64,0.64±0.21 vs 0.18±0.04,0.83±0.32 vs 0.06±0.02 mg/L,respectively,P<0.01 ).The plasma C5b 9 level was positively correlated with peak CK level (r=0.56,P< 0.001) and inversely correlated with the EF (r=-0.49,P<0.005).The plasma C5b 9 level of patients with KillipⅡ～Ⅳ was significantly higher than that of patients with KillipⅠ(1.14±0.41 vs 0.68±0.27 mg/L,P<0.01).Conclusion There was an activation of complement system after AMI and there was significant correlation between the degree of activation of complement system and the degree of myocardial damage.更多还原
