肺表面活性物质对盐酸吸入性肺损伤肺渗出的影响
Effect of Large Dose Pulmonary Surfactant on Exudation of Rabbit Lung Injury Induced by Aspiration of Hydrochloric Acid
【摘要】 目的 :探讨外源性PS对盐酸吸入性损伤家兔肺组织渗出的影响。方法 :健康家兔 2 0只复制盐酸吸入肺损伤模型后随机分为二组 ,A组 (n =1 0 )为PS治疗组 ,B组 (n =1 0 )为治疗对照组 ,气管内分别注入 37℃PS1 0 0mg/kg和生理盐水 (NS) ,对比观察PS治疗对肺水、BALF蛋白浓度及肺组织形态学改变。另设C(n =8)组 ,未行盐酸吸入 ,而在相应时间注入同等体积的NS作为PS疗效的对照。结果 :A、B组各肺水参数明显高于未损伤的C组 ,(P <0 0 1 ) ,A组肺水低于B组 ,但P >0 0 5 ;A组BALF蛋白浓度为 1 0 1 2 7± 1 39 7mg/L ,低于对照组的 1 394 7± 2 88 8mg/L ,P<0 0 1。肺组织形态学变化 :A组肺毛细血管充血、出血程度轻于B组 ,肺泡扩张程度好于B组。结论 :大剂量外源性PS治疗可减轻肺毛细血管充血、出血 ,部分减少蛋白渗出和肺水 ,对盐酸吸入性肺损伤起到一定的治疗作用
【Abstract】 Objective:To observe the effect of pulmonary surfactant (PS) on exudation of rabbit lung injury induced by aspiration of hydrochloric acid (HC1).Methods:Twenty HC1 acute lung injury rabbits were divided into groups A and B and treated with PS and NS respectively.We also had group C with no HC1 aspiration.The change of BALF protein concentration,lung water and morphology were observed.Results:The lung water in group A and B was higher significantly than that in group C.The BALF protein concentration of group A was significantly lower than that of group B and higher than that of group C.The changes of lung morphology were less in group A than in group B,but more than in group C.Conclusion:PS treatment has beneficial effect on HCl acute lung injury through improving lung inflation,partly reducing lung exudation and lung water.
