血清胸腺嘧啶核苷激酶的检测及临床意义

【摘要】 利用抗胸腺嘧啶核苷激酶 (TK)单克隆抗体 ,建立增强化学发光法 (ECLA) ,检测血清中的TK。在肿瘤病人中 ,肺癌的TK阳性率为 91 5 % ,直肠癌为 90 0 % ,胃癌为 90 6 % ,急性淋巴细胞性白血病 (ALL)、急非淋白血病 (ANLL)、淋巴瘤分别为 93 3% ,6 5 0 %和 91 6 % ;非肿瘤病人TK的阳性率为 4 2 %。 2 0 7例体检健康人群血清均为阴性 ,TK含量明显低于各肿瘤组病人 (P <0 0 0 1) ,而与非肿瘤病人无显著差异 (P >0 0 5 ) ;胃癌和直肠癌病人的TK阳性率明显高于癌胚抗原(CEA)的阳性检出率 (P <0 0 0 5 )。结论 :血清TK含量大于 5pmol/L可判定为异常 ;ECLA检测TK对ALL、淋巴瘤、肺癌、胃癌和直肠癌等有较高的诊断效果 ,将会成为肿瘤筛查、恶性程度和预后观察的工具。而且本法具有特异性好 ,结果易于观察且可长期保存的优点。

【Abstract】 Deoxythymidine kinase(TK)is a key enzyme involved in DNA synthesis.It can be used as a marker of cell proliferation.Enhanced chemiluminescence assay(ECLA)detecting serum TK is developed in the study.The positive rate in lung cancer patients is 91 5%,90 0% in rectum cancer,90 6% in gastric cancer,93 3% in ALL,65% in ANLL,91 6% in lymphoma;The positive rate in non tumor parients is 4 2%.207 normal human serum is negative,Whose TK values are lower than that of tumor groups(P<0 001),and not significantly differences from that of patients(P>0 05).The positive rate of TK in gastric and rectum cancer patients serum is higher than that of CEA(P<0 005).The conclusion think that the normal values of TK is less than 5 pmol/L,Meanwhile ECLA detecting TK can be used to monitor tumor,but has high positive rate in lung cancer,ALL,lymphoma,gastric and rectum cancer.It would also a useful tool for tumor effectiveness of treatment,tumor status and to predict prognosis.The experiment show that the assay is specificity and the result can be reserved lastly.
