化疗栓塞对肝细胞癌转移抑制基因nm23/ndpk表达的影响──介入治疗与肿瘤转移潜能研究之一
The Effect of Chemoembolization on nm23/ndpk Gene Expression of Primary Hepatocellular Carcinoma
【摘要】 目的 研究转移抑制基因nm 2 3/ndpk在原发性肝细胞癌 (PHC)中表达的意义以及不同化疗栓塞方法对其表达的影响。材料与方法 7种不同组织学类型的PHC共 98例 ,单纯手术 5 7例 ,4种介入治疗后Ⅱ期切除 41例。采用链霉亲合素生物素酶复合物 (SABC)免疫组化方法 ,检测各样本中nm 2 3/ndpk的表达 ,并与临床转移情况对照。结果 ndpk阳阴性者转移率分别为 41.2 %、80 .0 % (χ2 =12 .6 3 ,P <0 .0 0 5 ) ,介入治疗组及其A、B、C、D各组和单纯手术组ndpk阳性率分别为 82 .9%、85 .7%、6 2 .5 %、86 .7%、90 .9%、5 9.6 % (介入组分别与单纯手术组相比 ,χ2 值依次为 6 .0 8、0 .86、0 .0 5、3 .2 8、3 .94,P值依次为 <0 .0 2 5、>0 .2 5、>0 .5、≈ 0 .0 5、<0 .0 5 )。介入治疗使梁索型及Ⅰ级PHCnm 2 3/ndpk表达增高幅度最大。介入组与单纯手术组转移率分别为 48.8%、5 6 .1% (χ2 =0 .5 2 ,P >0 .2 5 )。结论 nm 2 3/ndpk高表达可抑制PHC转移 ,其可作为PHC预后评估指标。就nm 2 3/ndpk而言 ,化疗栓塞不促进PHC转移 ,且碘油、无水乙醇、明胶海绵多材料联合化疗栓塞优于单一材料栓塞和单纯化疗。
【Abstract】 Objective To study the significance of nm23/ndpk gene expression of primary hepatocellular carcinoma (PHC), and the effects of different chemoembolization therapy on nm23/ndpk.Materials and Methods Ninety-eight histologically verified PHC specimens were collected. The patients were treated with surgical resection alone (n=57) or second stage surgical resection after four kinds of chemoembolization (n=41). Strept avidin-biotin complex (SABC) immunohistochemical staining with multiclonal antibody against human nm23/ndpk was used to observe the ndpk in all specimens. The experimental results were compared with the surgical and clinical findings.Results The metastatic rate in ndpk(+) and (-) was 41.2% and 80.0%, respectively (χ 2=12.63, P<0.005). The positive rate of ndpk in interventional group, its subgroup A (chemotherapy alone), B (chemotherapy combined with iodized oil), C (chemotherapy combined with iodized oil and spongia gelatini absorbens), D (chemotherapy combined with ethanol, iodized oil and spongia gelatini absorbens), and surgical group was 82.9%, 85.7%, 62.5%, 86.7%, 90.9% and 59.6%, respectively. The metastatic rate in interventional group and pure surgical resection group was 48.8% and 56.1%, respectively (P>0.05).Conclusion Increased expression of nm23/ndpk will restrain PHC from metastasizing. It may be served as a prognosis-predictive marker. With respect to nm23/ndpk, chemoembolization fails to enhance the metastatic potential of PHC. A Combination of chemoembolization with ethanol, iodized oil and spongia gelatini absorbens is superior to pure chemotherapy and/or embolization with single material.
