组织谐波频移成像在华支睾吸虫病中的应用
Clinical value of tissue harmonic imaging in the liver and biliary system in clonorchiasis sinensis
【摘要】 目的:探讨组织谐波频移成像技术(THI)在华支睾吸虫所致肝胆声像改变特点和临床应用价值。方法:选择30例华支睾吸虫病患者进行普通二维成像和THI成像。观察华支睾吸虫病所造成的肝脏,胆囊,肝总管,左右肝管,肝内小胆管的损害,管腔内虫卵以及虫卵为中心结石的数量。结果:普通二维成像对华支睾吸虫的诊断符合率80.6%,THI为100%。华支睾吸虫所造成肝脏,胆囊,肝总管,左右肝管,肝内小胆管损害,管腔内虫卵,虫卵为中心结石数量显示,两组对比P<0.01有显著性差异。结论:华支睾吸虫主要造成胆系的损害,胆道系统为含液的非线性组织,是THI显像的最佳范畴。胆道壁与腔内胆汁,胆汗与虫卵以及以虫卵为中主的结石,管腔内胆汁与胆道后壁之间均存声阻抗差异,普通二维显像由于声波物理特性造成部分声学信息探测不到,THI显像弥补这一不足,为华支睾吸虫的诊断和疗效判定提供新的方法,也为THI在腹部其他疾病确定提供理论论据。
【Abstract】 Objective: To investigate the clinical value Tissue Harmonic Imaging (THI) in the liver and binary system in clonorc sinensis.Method: 30 cases of clonorchiasis sinensis were studied by two-dimensional ultrasonography and THI. Results: The result showed that the diagnosis accordance rate of THI in clonorchiasis sinensis was 100%, while two-dimensional ultrasonography was 80.6%. There was a signifant difference in the damage to the liver, binary, common hepatic duct, small bile ducts in the liver and lumen ova caused by clonorchiasis sinensis (p<0.01 ). Conclusions : THI is better than two-dimensional to judge the polypi number and peripheral exudate of biliary THI is a better method to judge the difference of the sound resistence in the affected liver, bile duct, gallstones, biliary and duct wall. It is suggested that the THI plays an important role in diagnosing and evaluating the therapy of clonorchasis sinensis compared with two-dimensional ultrasonography.
