【摘要】 目的 探讨基层医院应用内窥镜手术治疗慢性鼻窦炎和鼻息肉的疗效及影响手术疗效的相关因素。方法 从该院 1999年 3月～ 12月完成随访的 56例内窥镜鼻窦手术进行分析， 56例中Ⅰ型 29例 (51. 7％ )、Ⅱ类 22例 (39. 3％ )、Ⅲ型 5例 (9％ )。其中 17例在局部麻醉下完成， 38例在局麻辅以静脉强化麻醉下完成， 1例在全麻插管下完成。同期行鼻中隔矫正术 2例。术后随访 6～ 12个月，平均随访 8个月。结果 Ⅰ型治愈率 89. 7％，Ⅱ型治愈率 86. 4％，Ⅲ型治愈率 60％，三型之间治愈率差异有显著性，手术并发症发生率 12. 5％。结论 鼻内窥镜手术是适合在基层医院开展的治疗慢性鼻窦炎和鼻息肉的有效方法，手术疗效与临床分型及术者的技术熟练程度密切相关。更多还原

【Abstract】 Objective： To discuss the effect and its related cause of endoscope surgery in basic level hospital on chronic nasosinusitis and nasal polyp.Methods: To analyse 56 cases of endoscopic sinus surgery followed up by our hospital from March to December of 1999,29 cases of typeⅠ (51. 7％ )， 22 cases of typeⅡ (39.3％ ),and 5 cases of typeⅢ (9％ ) are founded.Among them,17 cases are under local anaesthesia,38 cases under local anaesthesia with vein potential anesthesia and 1 case under total anatsthesia intubation.2 cases of nasal septum epithesis are done homeochronously.The time for following－ up after operation is 6 to 12 months and its average time is 8 months.Results: The curative rate is 89. 7％ in typeⅠ ,86. 4％ in typeⅡ , and 60％ in typeⅢ ,Which shows notable difference among the three types.The incidence of operative complication is 12. 5％ .Conclusion: Endoscopic sinus surgery is an effective treatment for chronic nasosinusitis and nasal polyp in the local level hospital.The curative effect has close connection with clinical typing and operator’s skill.
