肺部薄层深呼吸CT

【摘要】 目的 :通过仰卧位与俯卧位薄层深呼吸CT扫描 ,观察肺叶内血流变化对肺叶通气会产生何种影响。方法 :对22名健康志愿者 ,取中或中下肺门层面扫描。测量仰、俯卧位呼、吸气各肺叶膨胀伸缩性及肺密度值。部分人还测量了肺叶面积。结果 :正常组肺叶呼气较吸气密度增加值是从下垂部到非下垂部逐级减少。正常组大多数下垂肺叶前后膨胀伸缩性大于非下垂肺叶(P<0.005)。肺叶膨胀伸缩性与肺叶面积变化基本一致。结论 :肺叶内血流变化可影响肺叶顺应性 ,进而又影响到肺叶通气性

【Abstract】 Objective: Thin-section CT was done in both supine and prone positions for the changes of blood affecting the lobal ventilation of the lungs. Method: 22 volunteers of 11 males and 11 females were selected for the study. Each scanning layer was made at the middle or middle-inferior hilum during full inspiration and respiration in both supine and prone positions. Elasticity and density of each lobe was then measured and cross-sectional area of each lobe was evaluated in some subjects. Results: Increase in lung attenuation during the exhalation phase gradually decreased from the dependent part to the independent part. The lobe elasticity in most dependent lobes was greater than that in the independent lobes ( P<0.005). The changes in lobe elasticity were basically consistent with that in the area of the lobe. Conclusion: It is implied that the changes of blood in a lobe can affect the compliance and then the ventilation of the lobe.
