保存人羊膜联合自体角膜缘移植治疗严重眼表疾病

【摘要】 目的:观察和评价羊膜移植在重建眼表面手术中的应用价值。方法:采用保存人羊膜联合自体或亲属健康眼的角膜缘移植治疗眼表面疾病48例52只眼。结果:术后随访60~92周,52只眼中,47只眼获得稳定的眼表面(除周边部有少量纤维血管翳外,中央部无新生血管及复发性上皮缺损发生),37只眼角膜透明或半透明,31只眼脱盲。结论:羊膜移植不是上皮移植,而是改良基底的手术,以保存人羊膜为基底膜,自体或亲属健康眼的角膜缘为上皮细胞来源,重建眼表面治疗眼表面疾病,初步结果显示是一种有价值的方法,其作用机制尚不清楚。

【Abstract】 Objective:To investigate and evaluate the applied value of preserved human amniotic membrane(PHAM) during surface reconstruction operation. Methods: Transplantation of PHAM combined with limbal epithelium of autoed or living- related was performed on 48 cases 52 eyes with severe ocular suface disorder(OSD). Results: With the observation period of 60-92 weeks,47 of 52 eyes were stable, and cental corneas were clear without neovascularization, pseudopterygium invasion, and persistent surface defect. 37 eye grafts were transparency or semi-transparency, and 311 eyes had been escaped blindness. Conclusion: The amniotic membrane transplantation (AMT ) is not arted as a surface transplantation.The amniotic is acted as a basis membrane. To use a conjunctival limbal autorgraft or a living related, conjunctival limbal allograft provides for a source of new growth epithelial to reconstruction ocular surface for patient with severe OSD. The therapeutic effects of the method is a very valuable and the operative mechanic should be stUdied baber. AMT seem to anti-inflammation, anti-fibrosis,regression neovascularizations, promotes normal conjunctival epithelialization, failitates migration of epithelial cells.
