口腔粘膜白斑发生发展中bcl-2、P21 wafl蛋白的表达

【摘要】 目的 :探讨口腔粘膜白斑发生发展中bcl 2及P2 1wafl蛋白的表达。方法 :利用免疫组织化学技术对 13例正常口腔糊膜、39例口腔粘膜白斑及 46例不同分级鳞癌的bcl 2、P2 1wafl蛋白表达进行研究。结果 :正常口腔粘膜bcl 2均呈阴性 ,不典型增生性白斑bcl 2蛋白阳性表达率为 5 2 .17% ,显著高于单纯增生性白斑 (18.75 % )及Ⅰ、Ⅱ级口腔鳞状细胞癌 (18.18% ,2 2 .2 2 % ,P <0 .0 5 )。正常口腔粘膜P2 1waf1蛋白表达均呈阳性。单纯增生性白斑P2 1waf1蛋白阳性表达率为 75 .0 0 % ,显著高于不典型增生性白斑 (2 6 .0 9% ) (P <0 .0 5 )及口腔鳞状细胞癌 (2 8.2 6 % )(P <0 .0 5 ) ,而P2 1wafl蛋白表达率在不典型增生性白斑和鳞癌之间差异无显著性 (P >0 .0 5 )。P2 1wafl阳性标本阳性细胞计数随组织恶性程度增加而有增高趋势。结论 :bcl 2蛋白表达在口腔粘膜恶性变的较早阶段起主要作用。P2 1waf1表达的下调是口腔白斑发生发展中的较早期事件 ,P2 1waf1表达率综合P2 1waf1阳性细胞计数在口腔粘膜白斑恶变潜能评价方面是一项有用的指标。

【Abstract】 Aim: To investigate the expressions of bcl 2 protein and P 21wafl protein in the progression of oral mucosa leukoplakia. Methods:13 cases of normal oral mucosa,39 cases of oral mucosa leukoplakia(LK) and 46 cases of squamous cell cancer(SCC) were examined by using immunohistochemical method.Results: None of normal controls showed bcl 2 positive staning.positire expression rate of bcl 2 protein was significantly higher in LK with dysplasia (52.17%) than that in LK with simple hyperplasia (18.75%, P <0.05)and that in grade I,Ⅱof SCC (18.18%, 22.22%, P <0.05) . All of normal oral mucosa showed P 21waf1 positive staining. P 21waf1 positive rate in LK with simple hyperplasia was 75.00%, which was significantly higher than that in LK with dysplasia (26.09%, P <0.05) and in SCC (28.26%, P <0.05). However, there were no significant differences of P 21wafl positive rates among LK with dysplasia and SCC( P >0.05) P 21wafl positive cell counting showed a tendency to increase with the malignant progression of oral mucosa lesions.Conclusion:The Expression of bcl 2 protein plays main role in which oral mucosa may transform to malignancy. The down regulation of P21wafl expression is an early event in occurrence and development of oral musosa LK. P 21wafl positive rate and P 21waf1 positive cell counting combined can serve as a useful indicator for the assessment of malignant potential of oral LK.
