肝脏海绵状血管瘤的磁共振成像诊断
MRI Diagnosis of Hepatic Cavernous Hemangioma
【摘要】 目的 分析 6 7例肝海绵状血管瘤的MRI表现 ,探讨磁共振成像 (MRI)对肝海绵状血管瘤的诊断价值。方法 6 7例病人均应用德国西门子公司生产的 1 0T超导MR机 (Impact)行肝脏扫描 ,选用SE序列横断面成像 ,分别施行T1WI、PDWI和T2 WI ,扫描层厚10mm ,间隔 0 2mm。 18例施行了静脉团注Gd -DTPA增强扫描。结果 6 7例病人肝脏MRI共检出 12 7个病灶 ,单发者 47例 ,多发者 2 0例。病灶大小介于 8mm× 6mm~ 110mm× 80mm。全部病灶均呈长T1和明显长T2 信号 ,T2 WI 112个病灶信号均匀 ,15个病灶信号不均 ,后者均大于 6 0mm× 5 0mm。 18例行增强扫描者 ,平扫发现 2 3个病灶 ,增强则发现 2 7个病灶 ,多发现的 4个病灶直径均小于 8mm。肝海绵状血管瘤典型增强表现为自周边向中心强化 ,可持续 2 0分钟以上。结论 MRI是诊断肝脏海绵状血管瘤以及同其它肝脏占位病变鉴别的有效手段。MRI增强扫描对发现肝脏较小海绵状血管瘤更有效。
【Abstract】 Objective To evaluate the diagnostic value of MRI for hepatic cavernous hemangioma (HCH), magnetic resonance imaging(MRI) manifestation of 67 patients with HCH was analysed.Methods MRI was performed with a 1.0T Siemens MR Unit in all cases, axial-transverse T 1 WI、 PDWI and T 2 WI of SE sequence were used, scanning slice thickness and gap of slices were 10mm and 0.2mm respectively. Intravenous bolus injections of Gd-DTPA were selected in 18 of 67 cases.Results 127 HCH lesions were found on MRI in 67 patients (solitary 47 patients, multiple 20 patients).Their dimensions were from 8mm×6mm to 110mm×80mm. Low signal intensity on T 1 WI and obvious high signal intensity on T 2 WI were inhibited in all lesions. 112 of 127 HCH lesions were showed as homogeneous signal intensity on T 2 WI and the other 15 inhomogeneous. Dimensions of these 15 lesions were more than 60mm×50mm. In 18 patients performed by Gd-DTPA contrast scanning, 23 lesions were found by plain MRI, 27 lesions were found by contrast scanning. These 4 lesions shown by contrast scaning were less than 8mm in diameter. Typical MRI contrast appearance of HCH was progressive contrast enhancement from periphery to center. Delayed time of contrast enhancement could be over 20 minutes.Conclusions MRI was a good choice for diagnosing HCH and differentiating from other hepatic neoplasms. Small HCH lesions were better revealed by means of contrast MRI.
