【摘要】 目的　比较rhTNF -NB与rhIL -2胸腔内联合注射 ,治疗恶性胸腔积液与单纯应用rhTNF -NB的疗效和毒副反应 ,寻求高效低毒治疗恶性胸腔积液的方法 ,以提高晚期癌症患者的生活质量。方法　对符合治疗条件的 5 0例恶性胸腔积液患者采用随机分组研究方法 ,观察联合应用 (方法 :先尽量抽取胸腔积液 ,先后将rhTNF -NB 2 0 0万单位用生理盐水 2 0ml稀释和rhIL -2 2 0万单位用生理盐水 2 0ml稀释注入胸腔。为减轻局部反应 ,可加 2 %利多卡因 10ml一并注入。每周 2次 ,2周为一疗程。如疗程中胸腔积液消失 ,即停止治疗。)与单独应用 (方法同上 ,每次仅注入rhTNF -NB 30 0万单位 )的临床疗效和毒副反应。结果　联合组有效率为92 0 % ,对照组为 44 0 % ,两组有显著性差异 (P <0 0 5 )。副作用有发热、寒战、恶心、呕吐、胸痛、乏力、血球变化及一过性肝肾损害等 ,两组无显著性差异。结论　rhTNF -NB与rhIL -2联合应用协同作用 ,疗效肯定 ,方法简单 ,优于单药使用更多还原

【Abstract】 Objective In the treatment of malignant pleural effusions, combining use of rhTNF-NB and rhIL-2 (combined group) was compared with single use of rhTNF-NB(single group) so as, to evaluate their therapeutic effect.Methods In 50 cases of malignant pleural effusions 25 cases were included in combined group in which rhTNF-NB 200×10 4 IU+rhIL-2 20×10 4 IU was injected into the pleural cavity; and 25 cases in single group, only rhTNF-NB 300×10 4 IU was used.Results The decrease rate of the total malignant pleural effusions was 92.00% in combined group and only 44.0% in single group, it differed significantly ( P <0 05) The side effects were fever,nausea, bosom ache, corpuscle transformation, etc.Conclusion Combining rhTNF-NB use with rhIL-2 appears to be very useful in treatment of malignant pleural effusions.更多还原
