【摘要】 目的 :探讨治疗髋臼骨折的方法以期提高疗效。方法 :对 1994年 1月至 1999年 12月收治的髋臼骨折 18例 ,按不同类型分别用螺丝钉、钢板、钢板 +螺丝钉或克氏针等以内固定治疗。结果 :随访 7个月至 6年 (平均 2年 9个月 )复位情况按 Judet等方法评估 ,达解剖复位者 12例 ,满意复位者3例 ,不满意复位者 3例。临床按 Matta评分 :优 10例 ,良 3例 ,可 3例 ,差 2例。术后 1～ 2年出现创伤性关节炎 2例 ,发生异位骨化者 3例 ,坐骨神经损伤 2例均恢复功能。结论 :早期手术是治疗髋臼移位骨折较为有效可靠的方法 ,手术进路的选择是复位成功的关键。早期不负重锻炼 ,透明质酸钠关节腔内应用利于功能恢复更多还原

【Abstract】 Objective:[WT5BZ]To research fora better method of treating acetabular fractures and improve operative effect.[WT5HZ]Methods:[WT5BZ]Eighteen cases of acetabular fractures were surgically treated from 1.1994 to 12.1999.Langenbeck Kocher and elongated iliofemoral incision were respectively taken basing on classification of acetabular fractures.The compressive screws,armor plates,and armor plates add screws or Kirchner nail were used for internal fixation.[WT5HZ]Results:[WT5BZ]12 cases anatomically reduced,3 cases were the better,2 cases were the poor in Judets criterion,all cases were followed up for 6 years and 7 months(average 2 years and 9 months),10 cases were the excellent,3 cases the good,2 cases had traumatic arthritis and 3 cases had heterotopic ossification.[WT5HZ]Conclusion:[WT5BZ]Earlier perod operation is a reliable and effective treating method for displaced acetabular fractures.Surgical approach is one of the most important factors of fracture reduction.Combination of Langenbeck Kocher with ilioinguinal incision or the elongated iliofemoral approach could meet the requirements of acetabular fractures.Early functional training of not weight loading and articular cavity makes use of sodium hyaluronate promotes recovery of function.
