【摘要】 目的 :采用切开复位折断式加压螺纹钉内固定 +带旋髂深血管蒂髂骨瓣移植治疗青壮年股骨颈骨折 ,减少骨折不愈合及股骨头缺血坏死的发生率。方法 :在解剖复位的基础上对青壮年股骨颈骨折采用 2～ 3根折断式加压螺纹钉内固定 ,同期植入带旋髂深血管蒂的髂骨瓣 ,术后遵循早锻炼 ,晚负重的康复的原则。结果 :本组 4 8例平均随访 4年 3个月 ,骨折愈合 4 7例 ,愈合率为 97.9% ,关节功能恢复优良率为 91.7%。结论 :折断式加压螺纹钉对股骨颈骨折固定稳定 ,操作简单、效果确切 ,同步行带旋髂深血管蒂髂骨瓣移植既提供了成骨的来源 ,又改善了股骨头的血供 ,促进骨折愈合 ,提高治疗效果更多还原

【Abstract】 Objective:[WT5BZ]To reduce no healing rate of transcervical fracture and anenic necrosis of capital ossis femoris in adults by internal fixation with pressor broken screw and transplanting iliac flap with deep circufles iliac vascular pedicle after replacement under operation.[WT5HZ]Method:[WT5BZ]Internal fixation with two or three pressor broken screws and transplanting iliac flap with deep circuflex iliac vascular pdeicle in the same time,which treat transcervical fracture in adult,according to principle of early exersise and later bearing burden after replacement in anatomy.[WT5HZ]Result:[WT5BZ]Forty seven cases out of forty eight were recovery ,cure rate was 97.9%,better recosery rate of joint function 91.7%.[WT5HZ]Conclusion:[WT5BZ]It is very stable to fix transcervical fracture with broken pressor screw ,which is very simple to operate on and very effective too,it provides origen of osteogenesis and improves blood supply with transplanting iliac flap with deep circuflex iliac vascular mean while it improves healing of fracture and cure effect.
