【摘要】 目的评价羊水指数 (AFI)在产前监护中的价值。方法研究 1998年 1月至 1999年 12月于我院进行产前羊水指数测定的 2 881例孕妇资料 ,就其中单胎、足月、无妊娠并发症和合并症 ,且于分娩前 1周内行AFI测定的 45 1例进行分析。AFI≤ 5 0mm、5 1～ 80mm和 >80mm分别为羊水过少、羊水偏少 (警戒值 )、羊水正常的标准。分别对三组围产儿结局进行比较。结果AFI≤ 5 0mm组不良结局围产儿较其余两组明显增加 (P <0 0 5 ) ;AFI =5 1～ 80mm与 >80mm组比较 ,P >0 0 5。提示AFI与AFD均可用于羊水过少的监测 ,但AFI具有更高的敏感性和特异性。结论AFI可用于羊水量的监测 ,同时可作为胎儿及新生儿预后的早期监测指标 ;AFI≤ 5 0mm常预示羊水过少和胎儿预后不良。更多还原

【Abstract】 Objective [WT5”BZ]To evaluate the value of AFI in antepartum monitoring.Methods[WT5”BZ] A retrospective review of all patients with entering antepartum examination was carried out at our hospital from January,1998 to December,1999.There were 451 women with singleton pregnancies,≥37 weeks’gestation and without complication,who underwent antepartum testing within l week of delivery and who were delivered at our institution were identified for our study.Amniotic fluid index(AFI)≤50mm was defined as oligohydramnios,AFI>80mm was considered normal and AFI of 51～80mm was reputed as “borderline”.[WT5”HZ]Results [WT5”BZ]There was a significantly increased incidence of adverse perinatal outcome (included intrapartum fetal distress,5 minute Apgar score <7,meconium-stained amniotic fluid intrauterine growth restriction and late decelerations) among the gravida with AFI≤50mm in comparison with AFI=51～80mm and AFI>80mm( P <0 05).There was no difference in perinatal outcome between AFI>80mm and AFI=51～80mm.The AFI and AFD have been used to assess the amniotic fluid.AFI has been validated as an accurate technique for it has a higher sensitivity and peculiarity than AFD.Conclusion The AFI has been validated as an accurate and reproducible technique for assessment of amniotic fluid volume and an early marker of perinatal outcome.AFI≤50mm was associated with oligohydramnios and adverse perinatal outcome.更多还原
