【摘要】 目的 研究Colles骨折治疗过程中的局部骨量变化及其临床意义。方法 于首次就诊及伤后 6周、 6个月、 1年测定 6 4名Colles骨折患者的第 2掌骨骨密度 ,并于伤后 1年进行功能评定。结果 Colles骨折患者的掌骨骨密度在骨折后 6周时即明显降低 ,骨折后 6个月时虽有所回升 ,但直至骨折后 1年仍未恢复原有水平 ,其统计学差异有显著性 (P <0 0 5 )或非常显著性 (P <0 0 1)。而骨折后 1年时掌骨骨密度与功能呈显著 (P <0 0 5 )或非常显著 (P <0 0 1)相关。提示骨密度低者功能也较差。不同类型骨折掌骨骨密度各项参数亦存在显著差异 (P <0 0 5 ) ,提示骨折程度严重者骨密度相应较低。结论 在Colles骨折治疗过程中发生的骨量丢失对骨折预后具有直接的影响 ,因此对于不同类型的骨折应采用不同的治疗方法 ,以减轻骨量的丢失 ;同时还应进行积极的功能锻炼促进骨量的尽快恢复

【Abstract】 Objective To investigate local changes in bone mass of the patients with Colles’ fracture and possible clinical relevance.Methods The radiographs of the second metacarpal in 64 patients with Colles’ fracture were measured for assessment of bone density at fracture,and six weeks,six months and one year after fracture.Functional results were evaluated at one year.Results Bone mass six weeks after fracture was significantly decreased,not returning to normal at one year though an increase in bone mass had been identified six months after fracture.The statistical differences were significant(P<0.05) or highly significant(P<0 01).At one year there were significant(P<0 05)or highly significant(P<0 01)correlations between bone mass indices of metacarpal and functional results,indicating that poor function is associated with lower bone density.There were also significant differences(P<0 05)between fracture patterns,suggesting that more severe fractures had a more pronounced bone loss.Conclusions Bone loss during the treattment course will have a direct effect upon the prognosis,so different treatment should be proposed.Active exercise should be made in order to improve the recovery of bone mass.
