【摘要】 目的：探讨肝癌患者外周血ＡＦＰ　ｍＲＮＡ的表达与肝癌转移的关系。方法：采用 ＲＴ－Ｎｅｓｔｅｄ ＰＣＲ技术，检测了 ９６例患者外周血中的 ＡＦＰ ｍＲＮＡ，并采用竞争 ＰＣＲ技术对其 ４７例阳性患者 ＡＦＰ ｍＲＮＡ的含量进行了检测。结果： ＡＦＰ ｍＲＡ阳性率分别为：对照组０，肝硬化组 １３． ３％，肝癌未发生转移组４２． ９％，局部转移组（肝内、门脉及胆管癌栓）６１．５％，远处转移组９０．９％。肿瘤直径≥５ｃｍ组ＡＦＰ　ｍＲＮＡ阳性率高于肿瘤＜５ｃｍ组（Ｐ＜０．０１）；血清ＡＦＰ≥４００ｎｇ／ｍｌ组ＡＦＰ ｍＲＡＡ阳性率高于＜４００ ｎｇ／ｍｌ组（Ｐ＜０．０１）；ＡＦＰ　ｍＲＮＡ阳性率与Ｃｈｉｌｄ分级无关（Ｐ＞ ０．０５）。阳性患者外周血中ＡＦＰ　ｍＲＮＡ含量分别为：肝硬化组（５．５×１０~４ ±４．５×１０~４） ｃｏｐｉｅｓ／ｍｌ，肝癌未发生转移组（１． ９０ × １０~６±１． ０２ ×１０~６） ｃｏｐｉｅｓ／ｍｌ，局部转移组（２． ５８×１０~６±２．３１×１０~６） ｃｏｐｉｅｓ／ｍｌ，远处转移组（２． ９８×１０~７±２．０８×１０~６） ｃｏｐｉｅｓ／ｍｌ。其中非肝癌组与肝癌组之间、肝癌无远处转移与远处转移组之间具有显著性差更多还原

【Abstract】 Objective: To investigate the relationship between AFP mRNA concentration in peripheral blood in patients with hepatocellular carcinoma (HCC) and tumor metastasis. Methord: 96 cases were included. The AFP mRNA concentration in peripheral blood were determined using RT-Nested PCR, and compitative RT-PCR technique were used in quatitative analysis for 47 positive eases. Results: The positive ratio was: control group 0, liver cirrhosis 13. 3%, HCC without metastasis group 42. 9%, HCC with local metastasis group 61.5%, HCC with distant metastasis 90. 9%. The results showed that significant differences between non-malignant and malignant group; HCC without distant metastasis and with distant metastasis group (P < 0.01). The presence of AFP mRNA seemed to be correlated with diameter of tumor and serum AFP value. The AFP mRNA concentrations in the positive were: liver cirrhosis group (5. 5 ×10~4±4. 5 × 10~4) copies/ml, HCC without metastasis group (1. 90 ×10~6 ±1. 02 ×10~6) copies/ml, HCC with local metastasis group (2. 58 × 10~6±2.3×10~6) copies / ml, HCC with distant metastasis (2. 98×10~7±2. 08 ×10~6 ) copies / ml.
