【摘要】 目的:探讨肝组织中Fas抗原表达和细胞凋亡的关系及其在乙型病毒性肝炎发病机理中的意义。方法:应用Fas抗原免疫组织化学染色和原位末端标记技术(TUNEL试验),原位观察84例乙型肝炎肝组织中Fas抗原表达强度和分布及凋亡细胞密度。结果:8例正常肝组织未见有Fax抗原表达,慢性重型肝炎的Fas抗原阳性表达率为100％,明显高于慢性肝炎中重度的66.7％(P<0.01);而慢性肝炎中重度的Fas阳性率则高于慢性肝炎轻度的36.7％(P<0.05)。Fas抗原阳性细胞主要位于肝组织碎屑状坏死区和小叶内灶状坏死区。慢性肝炎轻度和慢性肝炎中重度的凋亡细胞密度分别为7.96±4.61和17.90±13.16,经统计学处理具有非常显著性差异(P<0.01);重型肝炎的凋亡细胞密度为36.95±17.28,明显高于慢性肝炎轻度和慢性肝炎中重度(P均<0.01)。连续病理切片观察,凋亡细胞和Fas抗原分布有明显的同域性。结论:乙型肝炎肝组织中的Fas抗原和细胞凋亡与肝组织损伤有关,可能在乙型肝炎发病机理中起重要作用。更多还原

【Abstract】 Aim: To evaluate the role of the expression of Fas antigen and apoptosis in liver tissue in the pathogene-sis of viral heoatitis B. Methods: Fas antigen was determined by immunohistochemical method in 84 patients with viral hepatitis B. Apoptosis was detected in situ in these patients by terminal deoxynucleotidyl transferase mediated dUTP nick end labeling (TUNEL). 8 normal liver tissue were studied as control. Results:The expresson of Fas antigen in hepa-tocyte was not detected in nomal liver tissue. The positive rate of Fas antigen in severe viral hepatitis was significantly higher than that in chronic hepatitis of severe degree (P<0. 01): and the detection rate of Fas antigen was higher in patients with chronic hepatitis of severe degree than that in patients with chronic hepatitis of slight and mild degree (P 0. 05). Most of the Fas antigen positive cells distributed in areas with piecemeal or focal necrosis. The density of apoptot-ic cells in liver tissue both in severe viral hepatitis and chronic hepatitis of severe degree were significantly higher than that in chronic hepatitis of slight and mild degree (P<0. 01); and their density in chronic hepatitis of severe degree was much higher than that in chronic hepatitis of slight and mild degree (P<0. 01). Conclusion: The results suggest that the expression of Fas antigen and apoptosis in liver tissue of viral hepatitis were related to liver injury. The abnormal expression of Fas antigen and apoptosis may play important role in the pathogenesis of viral hepatitis.更多还原
