核素因子分析显像对原发性肝细胞癌诊断价值的研究
Diagnosis of early stage hepatocellular carcinoma with radionuclide factor analysis imaging
【摘要】 目的 利用因子分析显像对原发性肝细胞癌(HCC)进行定性、定位诊断研究。方法 对正常16 例,HCC 13 例,肝海绵状血管瘤21 例经肘静脉“弹丸”方式注入99m Tc 洗脱液370 MBq(体积< 1 m l)后,立刻以1 帧/5 s的速度连续采集36 帧,通过感兴趣区法扣除肝外影像,经图像放大,取28~32 个动态像素处理,提取肝实质相因子图像,肝动脉灌注相因子图像。结果 13 例HCC动脉灌注因子图像中11 例于占位区呈浓聚像,而正常人和肝血管瘤者未见异常浓聚,诊断效能评价,因子分析法对HCC的诊断灵敏度84.6% ,特异性100% 。结论 因子分析法对HCC的诊断具有较高的灵敏度和特异性,值得推广和应用。
【Abstract】 The purpose was to qualify and localize the lesions of hepatic cellular carcinoma(HCC) using radionuclide factor analysis imaging.Methods 16 normal volunteers, 13 patients with HCC and 21 with hepatic spongy hemangioma were injected 370 MBq 99m Tc04 elute. 36 frames were acqusited immediately at the rate of 5 frames per second with matrix 64×64, enlargement 1.5. The part out of liver was subtracted with ROI technique. After enlarging, 28~32 dixels were processed to extract hepatic artery phase, hepatocellar phase and their factor curves respectively. Results The results showed that the space occupying lesions in 11 out of 13 patients with HCC were visualized in the hepatic artery phase while that in the normals and the patients with hepatic spongy hemangioma were negative.Conclusions The sensitivity of HCC diagnosis was 84.6% and the speciality 100%. The factor analysis is sensitive and special for HCC diagnosis and has great value in clinical applications.
