椎间孔与椎间孔外腰椎间盘突出症的影像学诊断

【摘要】 目的 :探讨椎间孔与椎间孔外腰椎间盘突出症的影像学特点 ,评价各种影像学检查手段的诊断价值。方法 :对我院诊治的 4 3例 (其中手术治疗 34例 )椎间孔与椎间孔外腰椎间盘突出症的影像学资料进行分析。结果 :4 1例行CT平扫 ,正确诊断率 92 .7% ;2 0例行MRI,正确诊断率为 85 % ;5例行磁共振脊髓造影 (MRM ) ,2例行髓核造影 CT ,均获正确诊断 ;3例行脊髓造影为阴性结果 ;4例行CTM ,均能显示病变 ,但与CT平扫所见相同。结论 :结合相应的临床表现 ,CT、MRI、MRM对该病的诊断有较高的敏感性和准确性 ,三者有特征性的影像学表现 ,MRM可能是最敏感的手段。

【Abstract】 Objective:To analyze image characteristics of the foraminal and extra foraminal lumbar disc herniation(FELDH) and the diagnostic value of each image method.Method:The image materials of 43 FELDH cases in recent 6 years were reviewed.Result:Accurate diagnosis of FELDH was made with Computed Tomography (CT) scan in 38 of 41 cases (92 7%).Magnetic resonance image(MRI) in 17 of 20 (85%),Magnetic Resonance Myelography(MRM) in 5 of 5,discography enhanced CT in 2 of 2,Myelography ehanced CT(CTM) in 4 of 4,1 case of myelography got negative result.The typical image characteristic in CT is the soft mass which is found in the foraminal or extraforaminal area and had the same dense with the disc.In MRI the image of disc tissue extended up to foraminal area,the nerve root whose serial number was identical to that of the vertebra was involved.In MRM the relation between protruded disc and nerves (include superior and inferior nerve roots,cauda equina) can be identified clearly,that is its advantage.Conclusion:With accorded clinical manifestation,CT/MRI/MRM are the sensitive and accurate image methods for the diagnosis of FELDH.
