肝动脉和经动脉门脉造影螺旋CT的应用价值
The value of spiral CT in CT arterial portography and CT hepatic arteriography
【摘要】 目的 探讨螺旋CT 扫描在肝动脉造影CT(CThepatic arteriography , CTHA) 和经动脉门脉造影CT(CTarterial portography , CTAP) 中的应用价值。方法 分析50 例小肝癌灶( 直径≤3-0 cm)CTAP 和CTHA 的图像,并与双期增强扫描进行对照。结果 CTAP 病灶检出率为92-2 % ,CTHA 病灶检出率为90-1 % 。可检出0-2 ~0-5 cm 的微小癌灶。CTAP 和CTHA 均可出现非病理性表现,CTAP 灌注异常出现率为13-0 % ,CTHA 非病理性强化灶出现率为19-1 % 。结论 螺旋CT 动脉造影能显著减少造影剂用量,提高CT 图像质量,CT动脉造影病灶检出率及显示程度明显高于双期增强扫描。结合临床AFP 升高和碘油CT 表现,CT 动脉造影能提高微小肝癌的检出率。CTAP 和CTHA 结合分析,可减少假阳性率。
【Abstract】 Objective To evaluate the usefulness of spiral CT in CT arterial portography (CTAP) and CT hepatic arteriography (CTHA). Methods The CTAP and CTHA manifestations in 50 patients with small hepatocellular carcinoma (<3 cm in diameter) were analyzed and compared with those in bi phase enhanced CT.Results The detection rate of CTAP and CTHA was 92.2% and 90.1%, respectively. They could reveal tumors of 0.2 cm~0.5 cm in diameter. Both CTAP and CTHA gave false positive findings, such as perfusion defects in 13.0% of CTAP and non pathologic enhancement in 19.1% of CTHA.Conclusion With the use of spiral CT technique, the quantity of contrast material administrated in CTAP and CTHA can be considerably reduced, and the quality of CT images signficantly improved. The detection rate of CTAP and CTHA is higher than that of bi phase enhanced CT. Simultaneous use of both procedures may help decrease the false positive rate, and increase the sensitivity of diagnosis for small hepatocellular carcinoma.
