运动单位数目估测技术的临床应用研究
Research of motor unit number estimates in clinic practice
【摘要】 目的 研究运动单位数目估测技术 (MUNE)在神经肌肉疾病诊断中的作用。方法 采用MUNE中的“统计学方法”测定了 3 2例肌萎缩侧索硬化 (ALS)患者、16例多发性肌炎 (PM)患者和 5 2名健康对照者的左侧大鱼际肌的运动单位 (MU)数目 ,并对结果进行方差分析。结果 ALS组、PM组和健康对照组的MU数目分别为 :10 5± 15、2 2 0± 4 2和 2 2 9± 4 2 ( x±s) ,经统计学处理 ,第一组分别与第二组和第三组比较 ,差异均有显著性意义 (P <0 .0 5 ) ,第二组与第三组之间差异无显著性意义。结论 MUNE技术作为一种神经电生理方法 ,可用于神经源性疾病 ,如脊髓前角细胞病变等的临床诊断 ,并有助于与肌源性疾病的鉴别诊断。
【Abstract】 Objective To investigate the use of Motor unit number estimate (MUNE) in clinic practice. Methods In order to evaluate the methodology of MUNE, 32 cases with amyotrophic lateral sclerosis (ALS, groupⅠ), 16 cases with polymyositis (groupⅡ) and 52 normal subjects (group Ⅲ) were studied with the use of the statistical method of MUNE on the thenar muscles. Results The corresponding numbers of motor units were 105±15 ( ±s ) in the patients with ALS, 220±42 in the patients with polymyositis and 229±42 in the normal subjects. The difference of the results was significant between group Ⅰ and group Ⅱ or group Ⅲ. Conclusion MUNE is a new, non invade, objective, neuroelectrophysiological method to assess the function of motor neurons in the spinal cord. It is valuable to differentiate the neurogenic disorders from the myogenic ones.
