【摘要】 目的检测大肠息肉、癌及相应正常大肠粘膜组织ＣＤ４４基因ｍＲＮＡ表达，探讨该基因表达在大肠肿瘤进展转移中的意义以及癌组织上皮源性亚型与造血源性亚型比值（Ｅ／Ｈ）的临床意义。方法应用逆转录、多聚合酶链反应和Ｓｏｕｔｈｅｒｎ杂交技术，检测２６例大肠癌患者癌组织、正常大肠粘膜组织和７例大肠息肉ＣＤ４４基因ｍＲＮＡ表达。结果息肉、癌、转移淋巴结中上皮源性亚型及含Ｖ６、Ｖ１０外显子的变异型均明显表达，而正常粘膜组织中表达不明显。Ｄｕｋｅｓ分期Ａ、Ｂ期癌Ｅ／Ｈ为１４±１０，Ｃ、Ｄ期癌Ｅ／Ｈ为１４±０９；癌浸润肌层者Ｅ／Ｈ为１６±１０，浸润全层者Ｅ／Ｈ为１３±０９；有肝或淋巴结转移者Ｅ／Ｈ为１４±１０，无转移者为１４±１０，未发现Ｅ／Ｈ与肿瘤分期、有无转移、浸润深度有关（ｔ检验，Ｐ＞００５）。结论ＣＤ４４ｍＲＮＡ包括含有Ｖ６、Ｖ１０外显子的变异型在大肠肿瘤发生发展的早期已有表达，变异型的表达在大肠肿瘤发生发展中具有重要意义。Ｅ／Ｈ值的临床价值需进一步探讨。更多还原

【Abstract】 Objective To test the expression of CD44 gene in colorectal polyps, cancers in contrast to that of normal mucosae, and investigate its importance in the progression and metastasis of colorectal tumors. Methods The expression of CD44 mRNA was examined in cancerous tissues and adjacent noncancerous mucosae from 26 patients with colorectal cancer and in colorectal polyps from 7 patients by reverse transcription polymerase chain reaction and Southern blot hybridization. Results Strong expression of the epithelial form and variant forms containing variant exon 6 or 10 of the CD44 gene was detected in polyps and cancerous tissues, whereas it was very weak or not detectable in normal colonic mucosae. Significant correlations were not observed between the epithelial variant/hematopoietic variant (E/H) ratio and clinical features( t test, P <0 05). The E/H ratio was 1 4±1 0 in Dukes’ stages A and B compared with 1 4±0 9 in stages C and D. The E/H ratio was 1 6±1 0 in cancers infiltrating to muscular layer compared with 1 3±0 9 in cancers infiltrating through the colonic wall. The E/H ratio was 1 4±1 0 in cancers with metastasis compared with 1 40±1 05 in cancers without metastasis. Conclusions Expression of CD44 epithelial forms was significantly higher in colorectal tumor tissue than in matched normal mucosa. CD44 variant forms were expressed in an early stage of colorectal tumorgenesis. Variant forms sharing exon V6 or V10 may play a role in colorectal tumorgenesis and progression. The clinical value of E/H ratio in colorectal cancer needs further elucidation.更多还原
