【摘要】 目的了解慢性肾盂肾炎患者的骨代谢情况。方法对３１例慢性肾盂肾炎患者进行了有关骨密度（Ｗａｒｄ’ｓ、Ｎｅｃｋ、Ｔｒｏｃｈ、Ｌ２－４）、血清骨钙素（ＢＧＰ）、尿脱氧吡啶啉（ＤＰＤ）、肾小球滤过率（ＴＧＦＲ）、血β２－ＭＧ、尿β２－ＭＧ等项目的检查，并将结果输入计算机，进行ｔ检验及ｒ直线相关等统计学处理。结果患者组血ＢＧＰ（７．０４１±５．７４３ｎｇ／ｍｌ）、尿ＤＰＤ（６．０５８±２．２４５ｎＭ／ｎＭＣｒ）、Ｗａｒｄ’ｓ三角部位骨密度（０．８３８±０．２２３ｇ／ｃｍ２）与对照组相比（分别为３．３５４±０．７６５ｎｇ／ｍｌ、４．５９６±１．６２１ｎＭ／ｎＭＣｒ、０．９３６±０．０７６ｇ／ｃｍ２）经ｔ检验具有显著差异性（Ｐ＜０．０５或Ｐ＜０．０１）；而Ｎｅｃｋ、Ｔｒｏｃｈ、Ｌ２－４部位骨密度、血清Ｃａ、Ｐ、Ｍｇ、ＡＫＰ、ＢＵＮ、Ｃｒ等项检查，患者组与对照组无明显差异（Ｐ＞０．０５）；患者组Ｗａｒｄ＇ｓ三角部位骨密度与ＴＧＦＲ、血β２－ＭＧ、尿β２－ＭＧ具有明显相关性（ｒ系数分别为０．５６７、－０．５００、－０．３３４，Ｐ＜０．０５）。结论慢性肾盂肾炎患者，即使没有发展到慢性肾功能不全阶段，也已存在骨代谢异常，应及早采?更多还原

【Abstract】 In order to know the bone metabolism in 31 patients with chronic pyelonephritis,we measured the level of serum osteocalcin(BGP),bone mineral density (BMD) of femoral neck,Ward’s triangle and trochanter,urine pyridinoline (PyD),total glomerular filtration rate (TGFR),blood beta 2 microglobulin (β 2 MG) and urine β 2 MG.The results showed that the patients’serum BGP,urine PyD and BMD of Ward’s triangle were 7 041±5 743ng/ml,6 508±2 245nM/nMCr and 0 838±0 223g/cm 2,respectively,while the data from the control group were 3 354±0 765ng/ml,4 596±1.621nM/nMCr and 0.936±0.076g/cm 2 respectively;the difference was significant ( P <0 05 or P <0 01).But there were no significant differences between the two groups in the BMD of femoral neck,throchanter and lumbar spine 2 4,serum calcium,phosphorus,magnesium,alkaline phosphatase,blood urea nitrogen and creatine levels (P >0 05).There were significant correlations between BMD of Ward’s triangle and TGFR,blood β 2 MG and urine β 2 MG ( r =0.567,-0.500,-0.334,respectively, P <0 05).It is concluded that the bone metabolism is abnormal in the patients with chronic pyelonephritis even without chronic renal dysfunction;therefore,the physician should take therapeutic measures in advance to prevent the occurrence of ostoporosis.更多还原
