氟尿嘧啶持续滴注及放射治疗胸段食管癌
CONTINUOUS INTRAVENOUS INFUSION OF FLUOROURACIL BASED CHEMOTHERAPY AND RADIATION IN THE MANAGEMENT OF THORACIC ESOPHAGEAL CANCER
【摘要】 目的研究Ⅱ期胸段食管癌采用同时放化疗的可行性、疗效和毒副作用。方法32例病人第1天开始放疗,60Co食管胸前后二野加三野同中心,每天200cGy,每周5次,总量58~66Gy,病变位于胸上段加双锁骨上预防60CoDT50Gy。化疗:丝裂霉素(MMC)6mg/m2静注第2天;氟尿嘧啶(5FU)每天1g/m296h持续输液泵静滴,第2~5天,第29~32天;顺铂(DDP)75mg/m2静滴,第29天。结果第二周期全量化疗17例,14例因白细胞下降,剂量减半,1例未能完成第二周期化疗。放化疗的有效率为968%。同时进行放化疗的毒副作用较单独化疗或放疗增加,但能耐受没出现Ⅳ级毒性。结论本研究取得了近期疗效,说明同时放化疗治疗食管癌是可行的。
【Abstract】 PURPOSE To study the possibility,effects and toxicity of concurrent chemotherapy and radiation therapy in stage Ⅱ thoracic esophageal cancer. METHODS 32 patients received radiation of 200 cGy 60 Co per day using isocentric irradiation of two to three fields (5 times weekly).Total dosage is 58~66 Gy,prophylactic irradiation of 50 Gy of 60 Co for the bilateral supraclavicular fields was carried out for upper thoracic esophageal cancer. Mitomycin(6 mg/m 2) was administered intravenously on day 2,and 5 FU 1 g/m 2 per day as continous intravenous infusion for 96 hours on day 2~5 and day 29~32,and cisplatin(75 mg/m 2) on day 29 by intravenous infusion. RESULTS 17 patients received full dosage of chemotherapy and 14 received half dosage because of drop in white blood cells.Total response rate was 96.8%.Toxicity was increased in combined chemotherapy and radiation in as compared to simple radiation or chemothrapy.However,patients could tolerate the combined treatment and no Ⅳ grade toxicity occurred. CONCLUSION This study showed good stort term effects while toxicity is not severe,and concurrent radiation and chemotherapy is feasible.
