【摘要】 目的　分析20例原发上颌窦恶性淋巴瘤的病理类型及治疗方法对预后的影响。　方法　从1979～1997年,本院共收治20例上颌窦恶性淋巴瘤。其中男性15例,女性5例,Ⅰ期5例,Ⅱ期15例;全部病例均经病理证实,弥漫性B细胞14例,浆细胞性3例,淋巴母细胞性1例。治疗方法:12例在放射治疗前化疗,1～6疗程不等,多数为2～3疗程。另有10例在放射治疗后化疗。放射治疗均采用60Coγ线,常规放射,平均剂量559Gy(414～713)/23～36次/36～55天。16例用鼻前野+耳前野,2例为鼻前野+双耳前野,2例为双面颈联合野+鼻前野放射,9例采用楔形滤片,为30°或45°　结果　全组五年生存率为62%,男性与女性的生存率分别为51%和100%,Ⅰ期和Ⅱ期的五年生存率分别为80%和57%,用CHOP及COBDP方案优于未用化疗及其他化疗者。浆细胞性淋巴瘤的疗效较好。7例患者死于淋巴瘤播散,而无放射野内的复发。　结论　浆细胞性淋巴瘤的疗效较好,局部放射所用剂量已足够,提倡综合治疗更多还原

【Abstract】 PURPOSE To compare the effect of treatment modality and pathology for the maxillary lymphoma.METHODS 20 patients were treated from 1979 to 1997,male were 15 andfemale were 5. All patients were histologically approved, 14 patients were diffuse B cell lymphoma.Chemotherapy was given before the radiotherapy in 12 patients; and 10 patients were given after the radiotherapy.Radiotherapy were given to all patients with the average dose of 55.9 Gy(41.4 71 3) RESULTS The overall 5 year survival rate was 62%, and 100% for female,51% for male.The 5 year survival rate for satge Ⅰ and Ⅱ were 80% and 57% respectively. No patients recurred in the radiation fields, and 7 patients died from disease progress.The treatment results for patients who were given the regimen of CHOP or COBDP were better than the others.CONCLUSIONS The dose of radition is enough, and more intense chemotherapy was needed to treat these patients.更多还原
