【摘要】 目的:探讨上皮性膀胱肿瘤的CT分期和早期诊断。兼论肿瘤基底强化线的病理基础。材料与方法:经手术或活检证实的上皮性膀胱肿瘤53例,男47、女6例。81％在50岁～80岁之间,最小28岁。主诉血尿49例（占92％）伴排尿困难、膀胱激惹征各17例。50例手术切除或电切。病理结果除乳头状腺瘤,低分化鳞癌各1例外,其余均为移行上皮癌。可确定临床分期45例:T₁期4例、T₂17例、T₃20例、T₄4例。全部作CT平扫和增强扫描,部份追加延迟充盈相扫描。结果:上皮性膀胱肿瘤CT诊断依据是局限性肿物或不规则膀胱壁增厚,轻度到中度强化;显示肿瘤基底强化线的T₁4例中有3例、T₂17例仅2例、T₃、T₄未见。该线消失提示肿瘤已侵犯至外肌层。结论:（1）CT增强扫描是确诊本病的重要手段之一;（2）CT对本病临床分期虽有一定限度,但基底强化线存在与否对临床分期有所帮助。更多还原

【Abstract】 Purpose: To study CT staging in epithelial bladder neoplasm and discuss pathological fundamental of the enhanced basic line of tumor. Material and Methods: 53 cases, 47 males and 6 females, with epithelial bladder tumor confirmed by surgery or biopsy were studied. The age of 81% cases was 50 to 80, and the youngest is 28. The common complaint was hematuria (49 cases) and dysuria and bladder irritability (17 cases respectively). The pathological diagnosis is transmigrative epidermoid cancer except 1 case with papillary adenoma and another case with poorly differentiated squamous cell carcinoma. The clim-cal staging can be determined in 45 cases: T14 cases, T2 17 cases, T3 20 cases and T4 4 cases. Plain and enhanced CT scan were performed in all cases and delayed scan was performed in some cases in filling phase. Results: The CT diagnosis of epithelial bladder neoplasm is based on local mass or irregular thickening of bladder wall, mild to middle degree of enhancement. The enhanced basic line could be seen in 3 of 4 T1 cases and in 2 of 17 T2 cases, none could be seen in T3 and T4 cases. Absence of the line indicates tumor invading into out layer of muscle. Conclusions: Enhanced CT scan is one of important methods of diagnosing epithelial bladder neoplasm. The enhanced basic line is helpful to staging the disease.更多还原
