【摘要】 分析我院呼吸科近４年间慢性肺心病下呼吸道不动杆菌感染住院患者的临床特点、细菌培养及药敏试 验，探讨其致病特点。结果显示：不动杆菌感染的发病率较前增高，占同期肺心病下呼吸道感染的首位（２８．５７％， ３０／１０５例）。伴较严重的基础病和合并症，应用多种抗生素、激素或免疫抑制剂是引起不动杆菌下呼吸道感染的危 险因素。其临床症状重，混合感染率高（４０．００％），院内感染率高（２０．００％），病死率高（２６．６７％）。不动杆菌对多 种抗生素有不同程度的耐药性，对氨苄西林、头孢噻吩、头孢唑啉、头孢哌酮和呋喃妥因的耐药率为 ５０．００％～１００．００％，对庆大霉素、妥布霉素、哌拉西林、美洛西林、替卡西林、特美汀、头孢氨噻肟、头孢他啶、伊米 配能和环丙沙星的敏感率均在７０．００％以上。提示慢性肺心病下呼吸道不动杆菌感染的预后不良。应根据细菌培 养和药敏试验合理使用抗生素，积极治疗原发病。更多还原

【Abstract】 An analysis was made with the clinical manifestations, bacterial culture and antibiotic susceptibities to Acinetobacter infection of the lower respiratory tract in 30 cases with chronic pulmonary heart disease who were hospitalized in recent four yeals. Results showed that morbidity of Acinetobacter infection was higher than before, it was 28. 57 % among the total infections in the same period. The clinical manifestations were serious, the incidence of mixed infections was high(40.00%), the hospitalization-acquired infection rate was high(20. 00% ) and mortality rate was also high (26. 67% ). The strains of Acinetobacter were resistant to ampicillin, cephalothin, cefazolin, cefuroxime, cefoperazone and nitrofurantoin ranging from 50. 00% to 100. 00 %. The susceptibility was more than 70. 00 % to gentamicin, tobramycin, piperacillin, mezlocillin, ticarcillin, ticarcillin/CA, cefotaxime, ceftazidime, imipenam and ciproloxacin. The prognosis was not good. Successful therapy for the chronic pulmonary heart disease is based on bacterial culture and antibiotic susceptibilities in vitro and underlying diseases should be treated.更多还原
