儿童小脑萎缩和小脑发育畸形的CT诊断
CT Diagnosis of Cerebellar Atrophy and Cerebellar Malformation in Children.
【摘要】 目的:评价CT对儿童小脑萎缩和小脑发育畸形的诊断价值。材料与方法:选择资料完整的30例(男17例,女13例,年龄4~12岁,平均4.88岁)其中小脑萎缩24例,小脑发育畸形6例,均经CT扫描,1例Chiari Ⅰ型畸形还经CTM检查。结果:小脑萎缩性病变如小脑半球容积减少和/或小脑上池、沟增宽,桥小脑角池增宽,蛛网膜下腔增宽,第四脑室扩大等见于24例。小脑发育畸形病变如Chiari Ⅰ型或Dandy-Warker综合征,脑穿通畸形,Jonbert综合征等见于6例。结论:CT应用于儿科临床对小脑萎缩和小脑发育畸形一般能做出明确诊断。但是,不同病因所致的小脑萎缩可有相似的CT表现,因此,小脑萎缩的病因学鉴别诊断当需结合临床症状和体征,实验室检查及病理结果。
【Abstract】 Purpose. To evaluate the use of CT in the diagnosis of cerebellar atrophy and cerebellar malfor-mation in children. Materials and Methods:Thrty patients(17 male, 13 female, aged 4-12 yeats; mean age, 4. 88 years) in which include twenty - four patients with cerebeller atrophy and six patients with cerebellar malfroma-tion, underwent CT scan; one patient with Chiari 1 type underwent additional CTM examination. Results: Cerbcllar atrophy lesions such as decreased cerebellar hemisphere volume and/or widened supracerebellar cistern, grooves, widened cerebellopontine angle cistern, widened subarachnoid cavity, and enlarged fourth ventrcle etc were seen in 24 patients. Cerebellar malformation lesions sueh as Chiari I type or Dandy - Warker syndrome, porencephaly, and Joubert syndrome etc were seen in 6 patients. Conclusions. In general, application of CT in children’s clinic can accurately diagnose the cerebellar atrophy and cerebcllar malformation However, cerebellar atrophy originated from different causes likely reveal itsell as the same CT finding, and therefore to differentiate between them from the etiology had need combine CT findings with clinical symptoms, signs, laboratory examinations, and pathologic re-sults.
