老年糖尿病患者红细胞胰岛素酶活性研究

【摘要】 目的 探讨年龄与病程对Ⅱ型糖尿病患者的胰岛素降解速度的影响。方法 对老年Ⅱ型糖尿病患者49 例、非老年Ⅱ型糖尿病患者71 例以及正常对照60 例,检测了红细胞胰岛素酶活性(EIA)、空腹血浆胰岛素(FINS)、空腹血糖(FPG)和糖基化血红蛋白(HbA1c) ,并以FINS和FPG乘积的倒数作为胰岛素敏感性指数(ISI) 。结果 老年及非老年Ⅱ型糖尿病患者的EIA 显著高于正常对照组,其中,老年Ⅱ型糖尿病患者的EIA 和FPG 高于非老年患者,而ISI显著低于非老年患者。相关分析表明,2 组糖尿病患者的EIA 与病程和FINS呈显著负相关。结论 老年Ⅱ型糖尿病患者的红细胞胰岛素降解速度升高,而ISI下降,提示随着年龄增大和病程延长,Ⅱ型糖尿病患者的胰岛素抵抗逐渐加重。

【Abstract】 Objective\ To study the effect of age and duration on the rate of insulin degradation of erythrocytes in patients with type 2 diabetes mellitus(DM).Methods\ Insulinase activity of erythrocytes(EIA),fasting plasma insulin(FINS),fasting plasma glucose(FPG)and glycosylated hemoglobin(HbA 1c)were determined in 49 aged patients with type 2 DM,71 non-aged patients with type 2 DM and 60 healthy controls.Moreover,the reverse of FINS and FPG products (1/FINS×FPG)was used as the insulin sensitivity index(ISI).Results\ EIA was higher and ISI was lower in both of aged and non-aged patients than those in the controls.In aged patients,EIA and FPG were increased and ISI was decreased,as compared with non-aged patients.Moreover,EIA was inversely correlated with duration of diabetes,FINS and age in the two groups of patients with type 2 DM.Conclusion\ Insulin resistance will turn serious,accompanying the older age and the longer duration of diabetes.
