【摘要】 目的　探讨炎症介质白介素－ ８（ＩＬ－ ８） 在急性心肌缺血再灌注期间的变化及其意义。方法　（１） 用ＥＬＩＳＡ 方法测定２８ 例溶栓治疗的急性心肌梗死（ＡＭＩ） 患者入院即刻，再灌注１、４．５、１０ 、２０ｈ 血浆ＩＬ－ ８ 的变化。（２）在兔的缺血再灌注模型上检测血浆ＩＬ－ ８，缺血心肌组织中性粒细胞聚积，髓过氧化酶（ ＭＰＯ） 和丙二醛（ ＭＤＡ）的含量。结果　（１）１３ 例溶栓治疗后血管再通，１５ 例未通。两组溶栓前ＩＬ－ ８ 均显著高于正常对照组（ Ｐ ＜０．０１） ，并持续升高２４ｈ ，但两组间无显著性差异。（２） 兔缺血１．５ｈ 血浆ＩＬ－ ８、缺血心肌组织中性粒细胞ＭＰＯ 活性及ＭＤＡ 含量明显增高；当再灌注１ｈ 和４ｈ 时升高更显著（ Ｐ＜ ０．０５ ，或Ｐ＜ ０．０１） ，且ＩＬ－ ８ 的释放与ＭＰＯ 活性呈正相关（ｒ＝ ０．６６４ ，Ｐ＜ ０．０１）。结论　ＩＬ－ ８ 在心肌缺血和再灌注期间释放，并诱导了中性粒细胞致心肌的浸润与损伤，其在临床的意义有待进一步探讨。更多还原

【Abstract】 Objective To study the changes and significance of inflammatory mediator interleukin-8 (IL-8) during reperfusion of acute myocardial ischemia.Methods (1) In 28 patients with acute myocardial infarction (AMI) who received thrombolysis, plasma IL-8 by ELISA was measured at admission and 1,4\^5,10,20 hours after reperfusion. (2) In the rabbit model with ischemia reperfusion, similar measurements were performed as well as the accumulation of neutrophils\, myeloperoxidase (MPO) activity and malenodialdehyde (MDA) content in ischemic myocardium.Results (1) 13 cases developed reperfusion (group A) and 15 did not (group B). Plasma IL-8 level at admission were markedly higher than those of the normal controls (P<0\^01) and lasted to 24 hours after AMI, but there were no significant difference between group A and B. (2) In the rabbit model with myocardial ischemia 1\^5h, plasma levels of IL-8, the accumulation of neutrophils, MPO activity and MDA content significantly increased in ischemic myocardium, these were further increased in myocardial ischemia 1\^5h reperfusion 1h and 4h. In addition, IL-8 release were correlated with MPO activity (r=0\^664,P<0\^01).Conclusion IL-8 is released during reperfusion of myocardial ischemia. It might induce infiltration and injury of neutrophils for myocardium. Its clinical significance should be further investigated.更多还原
