【摘要】 目的　 老年糖尿病肾病 (DN)发生隐匿 ,为对其及早发现和治疗 ,我们测定了血清Ⅳ型胶原水平与DN早期的相关性研究 ,同时观察葛根素对血清Ⅳ型胶原及尿白蛋白的影响及临床治疗作用。　 方法 　老年糖尿病(DM )患者 46例 ,对照组 3 0例 ,测定血清Ⅳ型胶原及血液流变学 ;观察Ⅳ型胶原与病程、糖化血红蛋白 (HbA1c)、年龄、体质量指数的相关性 ;早期DN组给予葛根素 1个疗程治疗后 ,观察用药前后的尿白蛋白、Ⅳ型胶原 ,及血液流变学变化。　 结果 　老年DM患者Ⅳ型胶原水平明显升高 (P <0 0 5 ) ,早期DN组较正常白蛋白尿组Ⅳ型胶原水平明显升高 (P <0 0 5 )。　 结论 　Ⅳ型胶原水平是DN发生发展的重要标志之一。使用葛根素治疗前后可看到Ⅳ型胶原水平及血液流变学改善 ,尿白蛋白降低 ,因此葛根素对早期DN有治疗作用。更多还原

【Abstract】 To investigate the clinical significance of type Ⅳ collagen and as a new clinical marker in the evaluation for the aging incipient diabetic nephropathy and to examine the effect of Puerarin. Methods The concentration of serum type Ⅳ collagen was measured for 46 patients with type 2 diabetes mellitus and 30 aging persons as control.Twenty two patients with aging incipient diabetic nephropathy (urinary albumin excrete rate,UAER≥20μg·min -1 and <200μg·min -1 )received the treatment for Puerarin 400mg to be intravenous injected,one time every day for 14 days. Results The serum type Ⅳ collagen concentration was significantly higher in patients with type 2 DM,either presence or absence of urina albumin.Higher circulating levels of the type Ⅳ collagen is higher in cases with abnormal UAER than the normal UAER.It was tightly relationship with the course of disease,age,and HbA1c. Conclusions These results suggest that serum type Ⅳ collagen could be a useful marker for the early stage of diabetic nephropathy.Puerarin could inhibit the nonenzymatic protein glycation,then reduced the type Ⅳ collagen glycation which damaged the glomerular basement membrane.In addition,we found the Puerarin could improve the stickness of blood.It could decrease the UAER of incipient diabetic nephropathy.更多还原
