孕妇肝病对围产儿的影响

【摘要】 的探讨孕妇病毒性肝炎,妊娠肝内胆汁瘀积症(ICP)和妊高征肝损对胎儿的影响。方法随机检测353例病毒性肝炎,42例ICP和64例妊高征伴肝损害孕妇所生新生儿Apgar评分和出生时体重。结果1.病毒性肝炎与妊娠肝损,ICP与妊高征肝损孕妇新生儿Apgar评分及体重均无显著性差异(均P>0.05)。2.妊娠肝损血清胆红素浓度与新生儿Apgar评分及体重呈非常显著负相关(r=-0.230,r=-0.350,P<0.01),病毒性肝炎孕妇血清白蛋白与Apgar评分呈显著正相关(r=0.018,P<0.05)。结论病毒性肝炎与妊娠肝损对胎儿影响相似。妊娠肝损高胆红素血症可影响胎儿生长发育,病毒性肝炎孕妇血清白蛋白水平则与新生儿出生时状态有关。

【Abstract】 bjective To examine the effects of viral hepatitis in mothers,intrahepatic cholestasis of pregnancy(ICP),and liver lesion caused by hypertension of pregnancy on fetus. Methods Apgar score and birth weight were determined in the newborns born by 353 pregnant women with viral hepatitis,42 with ICP,and 64 with liver lesion caused by hypertension of pregnany. Results]1.There were no significant difference in Apgar score and birth weight between the newborns born by mothers with viral hepatitis and those by mothers with ICP and hypertension of pregnancy,and between the newborns born by mothers with ICP and those by mothers with hypertension of pregnancy (all P >0.05).2.Serum levels of bilirubin of mothers with ICP and hypertension of pregnancy were negatively correlated significantly with Apgar score and birth weight( r=-0.230,r=-0.350,P< 0.01),whereas serum concentration of albumin of mothers with viral hepatitis was positively correlated significantly with Apgar score( r=0.018,P <0.05). Conclusion Viral hepatitis in mothers might have a similar effect on fetus with ICP and hypertension of pregnancy.Serum levels of bilirubin of mothers with ICP and hypertension of pregnancy may influence the growth of the fetus,and serum concentration of albumin of mothers with viral hepatitis may be related to birth conditions of the fetus.
