心律平对心肌细胞钠钙通道的抑制效应

【摘要】 目的研究抗心律失常药物心律平(propafenone)对通道电流的作用。方法运用细胞内微电极、电压箱和膜片精技术从细胞动作电位、跨膜离子流和单通道三个水平研究心律平的作用。结果心律平对钠流、钙流有明确的抑制作用,减少晚钠通道电流爆发型模式的出现,并能抑制早期后除极的平台期振荡。结论心律平对心肌细胞钠通道和钙通道有明确抑制效应,为心律平应用于室上性和定性心律失常的治疗提供理论依据。

【Abstract】 ?0bjective The electrophysio1ogical of antiarrhythmic drug, propafenone, oncardiac channel current were studied. Methods The c0nventional microelectrode, voltageclamp and patch clamp techniques were employed on cardiac ventricular muscle, Purkinjefiber and isolated single myocyte in sheep, dog and guinea pig hearts. ResuIts Propafenonedecreases fast Na current, slow inward Ca current and suppresses early after depolarization.Popafenone also diminishes the occurrence frequency of the burst mode of Na channel.ConcIusion Propafenone suppresses Na channel and Ca channel in cardiac cells. Thesestudies offer a theoretical basis for the clinical practice in treatment of supraventricular andventricular arrhythmias.
