【摘要】 目的 :观察和探讨椎板截骨再植的椎管扩大成形术治疗腰椎管狭窄症的疗效。方法 :常规后正中切口 ,以L4～ 5 两节段病变为例 ,分别切断L3～ 4、L5 、S1棘间韧带 ,棘上韧带 ,于两侧小关节突内侧斜形 4 5°切断椎板 ,可将椎弓后侧部分整块取下 ,使椎管全部敞开 ,处理完椎管内的各种病变后 ,将截断的椎板以“后移再植”等 4种再植方式放回原处。结果 :按评级标准 ,优良率达 96 %。结论 :椎板截骨再植的椎管扩大成形术可以有效地预防术后腰椎不稳 ,顽固性疼痛 ,硬膜外神经根疤痕粘连。是一种治疗腰椎管狭窄症的理想方法更多还原

【Abstract】 Objective: To evaluate the effect on the treatment of the Lumbar spinal canal stenosis by the sectioned laminae and regrafing for spinal canal enlargement. Methods: we take L 4～5 region involved for example, First, resect interspinous and supraspinous Ligament respectively by the dorsal median incision, then cut off the laminae at inner bilateral small articular process and open the spinal canal through taking down the back parts of vertebral arch, finaly, The sectioned Laminae be placed in the original position often treating of all kinds of the pathologicall esions in the spinal canal. Results: The rate of fine result was 96% according to the evaluation criteria of curative effect. Conclusions: The secfioned laminae and regrafing for spinal canal enlargement can prevent effectively postoperative lumbar spina linstability obstinate pains and scar tissue adherent to dura and nerve root. It is an ideal methods for treatment of the lumbar stenosis. [更多还原
