【摘要】 目的：分析15mm以下周围型小肺癌的CT、HRCT表现特征及其病理基础。方法：回顾性分析59例15mm以下周围型小肺癌的CT、HRCT和病理表现。观察CT征象时，依据肺纵隔窗缩减率将肿瘤密度分为致密型、含气型和中间型三种。依据血管、支气管集束征的程度将其分成0～Ⅲ度。结果：本组15mm以下周围型小肺癌的CT和HRCT征象与较大肺癌比较有明显差别。毛刺、分叶、胸膜凹陷征等征象减少，支气管气相、血管集束征等征象有所增加。小肺癌的HRCT征象与组织类型和分化程度之间存在有一定联系。含气型，支气管气相等多见于细支气管肺泡癌和高分化腺癌。致密型，毛刺征和分叶征等征象多见于中低分化腺癌和其它组织类型的周围型肺癌。结论：①小肺癌的CT征象与较大肺癌比较有明显差别，在诊断上应引起注意。②小肺癌的CT征象与肺癌的病理类型及分化程度具有一定的联系。更多还原

【Abstract】 Objective: To analyze the CT featurs of small peripheral lung cancers less than 15mm in diameter and the relationshipwith pathological findings. Materials and methods: 59 cases of small peripheral lung cancers less than 15mm in diameter proved by operation and pathology were studied retrospectively. Results: The CT features of small peripheral lung cancers less than 15mm in diameter aredifferent from that of the larger lung cancers.Spiculation lobulation indentaton were decreased, and the sign of air - bronchogrambronchial-vessel concentration were increased in this study.The HRCT appearances are well correlated with histological types of the smalllung cancers. Air - containing type and air - bronchogram were seen more often in bronchioalveolar carcinoma and well - differentiated adenocarcinoma. But solid - density type and spiculation lobulation wer seen more of ten in poor - differentiated adenocarcinoma or other histological type lung cancers. Conclusion: 1, The CT features of small peripheral lung cancers less than 15mm in diameter were different fromlarger ones. 2,The CT features were well correlated with the histological types and the grades of differentiation.更多还原
