【摘要】 目的:研究rasp21、c-erbB-2、c-erbB-3、p53、nm23蛋白与细支气管肺泡癌(BAC)淋巴结转移的关系。方法:应用免疫组化LSAB法检测50例BAC(其中18例伴淋巴结转移)标本。结果:rasP21、c-erbB-2、c-erbB3、P53蛋白在伴或不伴淋巴结转移的BAC中的表达差异无显著性(P>0.05)。但在18例伴淋巴结转移的BAC原发灶内,rasP21、c-erbB-2、c-erbB-3、p53中两种及两种以上同时表达的有4例(22%),而在相应的淋巴结转移灶中则为8例(44%)。nm23/NDPK在BAC中的表达(34/50,78%)高于癌旁肺实质(P<0.01)。在伴淋巴结转移的BAC中nm23/NDPK表达率为56%(10/18),在无淋巴结转移的BAC中为91%(29/32),差别有高度显著性(P<0.01)O结论:肿瘤转移受多种基因及其表达产物的调控。nm23基因的表达不仅与BAC的发生有关,而且在BAC中起到转移抑制基因的作用。

【Abstract】 Purpose To study the correlation between ras p21, c-erbB-2, c-erbB-3, p53 and nm23 proteinand lymphatic metastasis of bronchiolo-alveolar carcinoma(BAC). Methods Using immunohistochemical tech-nique LSAB method, 50 patients with BAC (including 18 cases with lymphatic metastasis) were examined. Re-sults The frequency of ras p21, c-erbB-2, c-erbB-3, p53 protein was no significant difference in BAC with orwithout lymphatic metastasis(P > 0. 05 ). Of 18 primary lesions of BAC with lymphatic metastasis, 4 (22 % )primary lesions simultaneously expressed two and more than two oncogenes, 8(44 % ) metastatic lesions did so.The expression of nm23/NDPK in BAC (34/50, 78 % ) was remarkably higher than that in the lung parenchy-ma adjacent to the carcinoma (P < 0. 01 ). The positivity rate of nm23/NDPK in BAC with lymphatic metastasiswas 56 % (10/18 ), 91 % (29/32 ) in cases without lymphatic metastasis and an obvious association was found.Conclusion Tumor metastasis is controlled by many kinds of genes and their protein products. The expressionof nm23 not only relates to the development of BAC, but also acts as a metastasis suppressor gene.
