肝硬化患者尿液CGMP检测及意义
Clinical Significance of Urine CGMP in Patients with Liver Cirrhosis
【摘要】 目的:通过分析尿液环磷酸鸟苷(CGMP)水平与肝硬化(LC)患者血液动力学参数、神经内分泌激素活力、尿钠分泌的关系,探讨CGMP在LC高动力循环发生中的作用。方法:分别采用放射免疫分析法、二步生化法测定了15例正常人,12例代偿期LC患者,37例失代偿期LC患者血浆中肾素(PRA)、醛固酮(PAC)、心房利钠因子(ANF)、一氧化氮(NO)及尿液CGMP浓度。结果:与正常人相比,代偿期、失代偿期LC患者尿液CGMP水平和血浆PRA、PAC、ANF、NO浓度均明显升高,尿钠水平明显降低(P均<0.05)。相关分析发现,代偿期、失代偿期LC患者尿CGMP水平与全身血管阻力(SVR)、尿钠分泌呈负相关,与ANF、NO水平、心输出量(CO)呈正相关。结论:代偿期、失代偿期LC患者尿液CGMP明显升高;CGMP参与LC高动力循环及水钠潴留。
【Abstract】 Purpose: To investigate the role of urine cyclic guannsine monophosphate (CGMP) in the pathogenesis of liver cirrhosis (LC), the relationships were analysized among urine CGMP and hemody-namic parameters, neurohormonal activation, and urine sodium excretion. Methods: The levels of urine CGMP and plasma atrial natriurctic factor (ANF), plasma renin activity (PRA), plasma aldosterone (PAC), nitric oxide (NO) were determined with specific radioimmunoassay kits and two-step colorimetric assay respectively in 15 healthy subjects, 12 patients with compensated LC and 37 patients with decompen-sated LC. Results: The levels of urine CGMP in patients with compensated LC and decompensated LC were significantly enhanced (P<0.01). The levels of urine CGMP in patients with compensated LC and decompensated LC were inversely correlated with systemic vascular resistance(SVR) and urinary sodium excretion; where as the levels of urine CGMP were correlated with the levels of plasma ANF, NO, CO. Conclusions: The levels of urinary CGMP were enhanced in both compensated and decompensated LC. The data above suggested that increased CGMP generation may be related to the initiation and maintenance of hyperkinetic circulation in LC.
