硝酸甘油介入99mTc-M IBI静息态心肌断层显像估测冠心病存活心肌的临床意义

【摘要】 目的:探讨含服硝酸甘油( N T G)介入99m Tc甲氧基异丁基异晴( M I B I)心肌断层显像在心肌存活估测中的价值。方法:54 例冠心病患者,其中陈旧性心肌梗死( O M I)、不稳定型心绞痛( U A)、稳定型心绞痛( S A)各18 例。先行静息态99m Tc M I B I心肌断层显像,48 h 后在静脉注入99m Tc M I B I前 5 m in 舌下含服 N T G 0.5 m g,然后再行静息态心肌断层显像。结果:54 列患者静息态心肌断层显像有168 个节段放射性分布异常, N T G 介入后有71 个节段灌注改善(42.3% )。 U A 组有8 个心肌节段为不可逆性灌注缺损, S A 组无不可逆性灌注缺损的心肌节段。结论:含服 N T G 能明显改善静息态99m Tc M I B I时心肌存活的检测效果,对判断病情和指导治疗有一定价值。

【Abstract】 Objective:To evaluate the clinic value of combination of rest 99m Tc MIBI myocardial tomography with nitroglycein (NTG) sublingual administration in assessing myocardial viability.Method:Routine rest 99m Tc MIBI myocardial tomography imaging was performed for 54 patients with coronary heart disease (18 with old myocardial infarction,18 with unstable angina and 18 with stable angina),and 48 hours later,NTG was administrated sublingually ( 0.5 mg) 5 minutes before the trace injection,and then rest mycoardial imaging was performed again.Result:54 patients had 168 segments of hypoperfusion in rest imaging,71 segments had improvement of perfusion after NTG administration ( 42.3 %).8 segments were identified as irreversible perfusion defects in patients with UA,and without in SA.Conclusion:Our data demonstrated that the administration of NTG before rest 99m Tc MIBI SPECT might be a useful approach for predicting myocardial viability,be helpful for assessing patients′s condition and direct treatment.
