癫痫患者经颅磁刺激运动诱发电位的研究

【摘要】 目的 探讨癫痫患者经颅磁刺激(TMS)运动诱发电位(MEP)对癫痫的诊断价值。方法 对35例癫痫患者(分治疗组与未治组)和30 例正常对照组的脑运动皮质和第7 颈椎水平神经根进行单脉冲磁刺激,引出MEP。结果 与对照组比较,两组患者的皮质阈强度均增大,治疗组增大更明显(P< 0.01);未治组的皮质潜伏期和中枢运动传导时间缩短,治疗组则无明显改变;三组的周围潜伏期无明显差异。癫痫类型、疗程、联合用药与否以及脑部病灶侧别对MEP无明显影响。结论 检测TMS的MEP可以协助诊断癫痫,指导癫痫合理用药。

【Abstract】 Objective To investigate the diagnostic value on motor evoked potentials (MEP) of transcranial magnetic stimulation (TMS) in the pationts with epilesy.Methods MEP induced by single pulse TMS were recorded in 35 patients with epilepsy (including 9 patients without antiepileptic treatment and 26 patients with antiepileptic treatment) as compared with 30 normal controls of their brain motor cortex and the seventh cervical vertebra level nerve root.Results As compared with the controls, the cortical stimulation threshold intensities were markedly elevatedin the two groups and the treatment group was more remarkable ( P <0.01), the cortical latencies and central motor conduction time ( CMCT) tended to be decreased in untreatment group, no marked change in the treatment group. Peripheral latencies were not significantly changed in all of the patients. The duration of antiepileptic treatment, the side of cerebral lesions ,the type of the seizures and the combined administration of antiepileptic drugs did not affect MEP at all.Conclusion MEP induced by single pulse TMS might be valuable for assisting the diagnosis of epilepsy and directed to take medicine rationally.
