脑动静脉畸形电子束CT三维血管造影的临床应用
Clinical Application of 3D Electron Beam CT Angiography in Intracranial Arteriovenous Malformation
【摘要】 目的:探讨电子束CT血管造影(EBCTA)在诊断脑动静脉畸形中的价值与限度。材料与方法:选择17例不同部位、不同类型的脑动静脉畸形进行EBCTA和DSA对照研究,采用最大信号强度投影(MIP)和表面遮盖法(SSD)进行重建。结果:EBCTA能100%显示瘤巢,MIP法供血动脉显示率为93.9%,SSD法供血动脉显示率为87.8%,引流静脉两者显示率均为85.7%,对于颈外动脉供血和小型AVM的供血动脉以及引流至海绵窦和深组静脉的情况显示欠佳。结论:EBCTA是诊断脑动静脉畸形的有效手段,但在显示脑动静脉畸形的血流动力学特征方面具有一定限度。
【Abstract】 Objective: To evaluate 3D-EBCTA in diagnosing intracranial arteriovenous malformation. Materials and Methods: Seventeen casesof intracranial arteriovenous malformation, in different locations and with different grades, underwent 3D-EBCTA and DSA. The images were reconstructed with MIP and SSD. Results:All niduses were demonstrated on 3D-EBCTA, and the feeding arteries were revealed in 93. 9 % on MIP and87. 8% on SSD, while drain veins were shown in 85. 7 % on both MIP and SSD. EBCTA had some limitations in demonstrating smaLll AVM, feedingarteries originated from extemal carotid, and drain veins leading to deep veins or cavemous sinus. Conclusion: 3D-EBCTA is a helpful means for thediagnosis of intracranial arteriovenous malformation, but it has some limits in showing dynamic features of AVM flow.
