小脑蚓部肿瘤的CT诊断(附95例分析)
CT Diagnosis of Cerebellar Vermis Tumors: An Analysis of 95 Cases
【摘要】 目的: 提高小脑蚓部各肿瘤CT 诊断的正确性。材料与方法: 95 例经手术、病理证实的小脑蚓部肿瘤,男性59 例,女性36例,术前均行CT 平扫及增强扫描。结果: 成神经管细胞瘤41 例( 男∶女= 21∶20 ,平均年龄17 .4 岁) ,CT 平扫以略高、等或略低混杂密度为主,增强后肿瘤可均匀或不均匀强化。星形胶质瘤29 例( 男∶女= 19∶10 ,平均20 .8 岁) ,CT 平扫以略低、低混杂密度为主,增强后可轻度、不规则强化。室管膜瘤10 例( 男∶女= 8∶2 ,平均23 岁) ,平扫以等、混合密度为主,增强多有均匀强化,常伴脑积水。成血管细胞瘤9 例( 男∶女= 5∶4 ,平均40 岁) ,平扫可分实质性和囊变两大类,增强后实质性肿瘤呈明显均匀强化,囊变者为大囊及附壁明显强化的小结节。另外少枝胶质瘤1 例,转移瘤3 例,脉络丛乳头状瘤1 例,海绵状血管瘤1 例。结论: 根据患者的年龄及CT表现有助于小脑蚓部肿瘤的鉴别诊断。
【Abstract】 Objective: To improve CT diagnostic accuracy for cerebellar vermis tumors.Materials and Methods:Plain and enhanced CT scanning were performed in 95 patients with surgically and pathologically proved cerebellar vermis tumors.Results:Medulloblastoma (n=41) mostly demonstrated slightly hyperdensity or inhomogenous density on plain CT scan, and showed homogenous or inhomogenous enhancement after contrast injection. Astrocytoma (n=29) was predominantly inhomogenously low density on plain CT scan and inhomogenously enhanced after contrasted. Ependymoma (n=10) was mainly iso or inhomogenous in density on plain CT scan and homogenously enhanced on post contrasted CT scan. All cases with ependymoma accompanied hydrocephalus. Hemangioblastoma (n=9) could be cystic mass (n=7) or solid mass (n=2) on plain CT scan. On post contrasted scan, the cystic hemangioblastoma showed a large cyst with marked enhanced nodules on the wall, while the solid lesion revealed significant homogenous enhancement and enlarged drain veins. Besides, oligodendroglioma (n=1), metastases (n=3), papilloma of chorial plexus (n=1) and cavernous hemangioma (n=1) were found in this study.Conclusion: Patient’s age and CT findings are helpful in making differential diagnosis for cerebellar vermis tumors.
