【摘要】 目的：探讨游离型腰椎间盘脱出症合并马尾神经损伤的发病率，分析临床症状、体征与影像学检查的重要意义。方法：４４例病人全部做ＣＴ扫描，诊断可疑者进行脊髓造影ＣＴＭ检查、脑脊液的检验。并且与马尾神经肿瘤进行了严格的鉴别。结果：马尾神经功能完全恢复３３例，不完全恢复１１例。恢复所需时间为１５～２年，唯双下肢瘫痪及性功能障碍恢复所需时间较长并且功能不理想。结论：手术后效果取决于马尾神经损伤的严重程度，对于较大整块脱出物进入蛛网膜下腔者预后最差。正确诊断、及时手术对于预后极为重要更多还原

【Abstract】 Aim:To estimate the incidence of free intervertebral disc cartilage protrusion complicated with cauda equina injury and analyse the importance of clinical symtom,physical sign and image examination.Methods:Computed tomography was performed on all of the 44 cases.For those with a doubtful diagnosis,computed tomographic myelography and cerebraspinal fluid testing were taken,Strict differential diagnosis was made to exclude tumor of cauda equina.Results:Full recovery for the function of cauda equina was obtained in 33 cases,partial recovery in 11 cases .The recovery time ranged from 1.5 years to 2years.Time for recovery of paralysed lower extrimity and sexual disorder was relatively longer with poor result.Conclusion:The effect of operation depends on severity of original injury.For those patients with a large piece of cartilage protruded into subarachnoid cavity,the prognosis was poor.Correct diagnosis and timely operation are very important if a good prognosis is expected.更多还原
