阔筋膜张肌的应用解剖
APPLIED ANATOMY OF TENSOR FASCIA LATA
【摘要】 目的:评价阔筋膜张肌的功能并为临床应用提供形态学基础。方法:研究人阔筋膜张肌的大体形态、肌纤维型分布、肌构筑学特征和肌梭分布密度。结果:Ⅰ型肌纤维占 54. 5%,肌重 44. 3g,生理横切面积 3. 7 cm~2,羽状角θ为0度,平均肌梭密度 6. 48个/g。结论:阔筋膜张肌是一块力弱的带状肌,与臀大肌共同组成平行肌组,在髋、膝关节的角度变化时起到运动监测器的作用,以唤起臀大肌的动作。作者假设它与缝匠肌、股薄肌共同组成三条运动监测带,在静力和动力状态下协助维持身体平衡。临床应用阔筋膜张肌和髂胫束作为覆盖物治疗缺损是可行的,而用该肌作肌移植则其肌力难以匹配。
【Abstract】 Object:To evaluate the function of the tensor fascia iata and provide an anatomic basis for the clinical application. Methods: The gross anatomy, muscle fiber types composition, architectural features and muscle spindle density of the tensor fascia iata were studied in cadavers. Results: The fibers, muscle weight, cross-sectional area of typeⅠ were 54. 5%, 44. 3±17. 9 g, and 3. 71. 5 cm~2 respectively. The pennate angle was 0 degree. The muscle spindle density was 6. 48 per gram in average. Conclusion: The tensor fascia iata was a strap muscle with weak strength. When the angles of hip and knee joints changed the tensor fascia iata seemed to compose a parallel muscle combination (PMC) with the gluteus maximum, and act as a kinesiological monitor so as to recruite the contraction of gluteus maximum. We suggested that the tensor fascia iata, sartorius and gracilis formed a kinesiological monitor of triangular bands to keep the body’s balance undev both static and kinesic conditions. The tensor fascia iata and ilio-tibial band were considered to be feasible in clinical use, but could not be used to replace a paralyzed muscle as a transplant.
