股骨颈骨折内固定物的力学对比及疗效分析

【摘要】 为临床上选择合理的内固定器材提供理论依据。16具成年新鲜股骨标本随机分成四组,制成股骨颈骨折,分别植入四种内固定物并进行生物力学测试。测试数据经统计学处理分析表明,三枚加压螺纹钉的刚度和强度最大且具有较强的抗剪力、弯应力及抗扭力。单一空心加压螺钉次之,三刃钉较差。临床随访资料表明,三枚加压螺纹钉的疗效仍居首位。认为三枚加压螺纹钉呈三角形立体植入股骨颈内,具有动性和静力性的加压作用,最大限度地减少应力遮挡效应,其固定作用更加牢固。

【Abstract】 To provide theoretic basis for rational choosing of internal fixation in clinical use through biomechanical experi-ment and comparative analysis of femoral samples after internal fixation, sixteen cases of femoral neck fracture, which were produced on adult fresh femoral samples, were divided into four groups at random. After different internal fixations were used separately for the fracture, biomechanical measurements were respectively made in the samples.The measure-ment data, showed that three-compression - screws were the best in rigidity and strength, single - hollow - compres-sion - screw was the second and trif in nail was poor (P < 0.01), and that three - compression-screws had better anti -shearing stress, bending strength and anti-torsion force. According to clinical data curative effect of three-copmression - screws was the first in these four internal fixations. Three-compression-screws, which fix femoral neck in the shape of triangle and three dimensions, accord with biomechanics, have dynamic and static compression effect, it reduce the stress shelding effect as possible and make the fracture fragments contact closer.
